With the growth of aging population,it is social focus for the elderly to spend a happy and healthy aged years.The provision of care services for meeting health demands of the elderly is a core part in aging business.The assessment of the elderly care in terms of physical condition does not only accurately reflect the demand of elderly care services,but also reasonably classify elderly care grades in order to take foundation for the provision of elderly care services.At present,assessment of care for the aged in China has not yet formed a unified standard and faced various problems: multiple assessment criteria coexisting,with wide content gaps,unscientific assessment indicators and large non-essential indicators;lack of assessment indicators that are based on the physical abilities of the elderly.According to the physical function and status of the elderly,they needed to know and judge their care needs in a timely manner;it was not match with the care services for the elderly.Therefore,it is necessary to build an assessment indicator system for elderly care based on the basic ability and status of the elderly,and to establish an assessment tool that matches the content of care services and the levels of care.ObjectiveBased on the basic abilities and status of the elderly,this study established an assessment index system for elderly care and determined the weight of each indicator.Based on this,a questionnaire was designed to measure the of the index system;and index system was used to specifically formulate care services for the elderly and build graded elderly care mode,a reasonable division of the elderly care levels.According to the above research process and results,reasonable suggestions for improving the development of endowment services are proposed.MethodsThrough the literature review and expert interviews,we initially selected the indicators of the basic abilities of the elderly,and identified the indicators of self-care ability,cognitive ability,basic health status,disease status,nutritional status,and psychological state.By the Delphi method to further determine,the representative indicators were confirmed in the six dimensions,and by analytic hierarchy process the weights of the indicators were calculated;By questionnaire,the index system was determined;By random sampling methods elderly people with different elderly care modes were extracted in Shanghai.It conducted a questionnaire survey.Descriptive statistical methods were used to statistically analyze the overall status of elderly people and the demands of elderly care services.Descriptive analysis and test for homogeneity of variance were used to describe the elderly care service items that were matched by the assessment index system.A hierarchical model of care for the elderly was constructed through analytic hierarchy process and rank-sum ratio method.The inspection level is 0.05.Results1.Establishment of the assessment index system for elderly care(1)This section has constructed six dimensions: self-care ability,cognitive ability,basic health status,disease status,nutritional status and mental state as the index dimensions representing the basic ability and basic state of the elderly,and confirmed 74 second level indicators.(2)A total of two rounds of Delphi method were consulted.Two rounds of expert consultation had high expert consultation coefficient,authority coefficient and coordination coefficient.The Kendall coefficient increased.The results of the consultation were credible and statistically significant(P<0.05).(3)Analytic hierarchy process determined six dimensions weights of each index.All the judgment matrices were consistent with the consistency test,and the results can be used to calculate the grading mode of care services.2.Questionnaire correction index systemThe questionnaires were compiled in strict accordance with the evaluation indicators.All the indicators had corresponding questions in the questionnaires.A total of 131 elderly people were surveyed,including 45.04% of males and 54.96% of females;the average age of the surveyed subjects was 85.98±6.791;community pension was 14.5%,and institutional pension was 85.5%;respondents' care grades ranged from secondary care to grade 6 The proportion of the population was 9.92%,9.16%,19.85%,28.24%,and 32.82%,respectively.3.Matching and reforming care servicesAccording to assessment index system,the assessment index,and care services,the care service content was reintegration and determined,and ultimately five care service types were determined including basic ability,health care,clinical care,nutritional care,psychological care with a total of 48 health care service items.4.Preliminary determination of the old-age mode and the construction of an old-age care classification mode(1)According to self-care ability and cognitive ability,we decided the elderly care modes,which were initially divided into four modes: home care,home care+ community care,community care and institutional care.(2)The index weight was used to calculate and divide the care classification,and the classification standard was determined.The use of the analytic hierarchy process and questionnaire data,calculate the formula of the T=A1*0.3398+A2*0.3398+A3*0.1592+A4*0.0762+A5*0.0344+A6*0.0507,and the level of care was divided into 4 grades and assessment index system weights was calculated by the questionnaire and the weight of each index partition.Countermeasures and Suggestions(1)Building an assessment index system based on basic abilities and basic status.(2)Results matched the care service and the nursing care mode classification,and enhance the practicability.(3)Application of the objects is more extensive and the universality is improved.(4)Meeting the dual assessment requirements of "access" and "sustainability".(5)Key to guarantee the quality of care service.
