Clinical Effect Observation About Dachengqi Decoction In The Treatment Of Severe Acute Pancreatitis

Objective: Through the Dachengqi decoction treating Severe Acute Pancreatitis of retrospective analysis and prospective study,summarizing the general clinical data and test data,doing relevant laboratory tests to observe the Dachengqi decoction treatment of Acute Severe Pancreatitis clinical curative effect,and discuss the relevant mechanism of action,Show the advantage of combining traditional Chinese and western medicine treatment of severe acute pancreatitis,provide experience and basis for clinical popularization and application for the future.Methods: 1,retrospective analysis: we collected 70 patients with SAP of the First Affiliated Hospital of Dalian Medical University from January 2003 to January 2015 to doing retrospective analysis.70 patients were randomly divided into observation group for 35 cases and treatment group for 35 cases.We adopted routine western medicine treatment in both groups,observation group on the basis of treatment with the Dachengqi decoction through gastric tube influx or enema,and counted the general clinical data and test data of patients.2,prospective study:(1)we collected 15 cases of SAP patients who were received by the First Affiliated Hospital of Dalian Medical University from January 2016 to December 2016 and randomly divided into the treatment group for 7 cases and observation group for 8 cases.We adopted routine western medicine treatment in both groups,observation group on the basis of treatment with the Dachengqi decoction through enema,and counted the general clinical data andtest data of patients.And used flow cytometry to detect the apoptosis situation of neutrophil.(2)We extracted patients' peripheral venous blood and extracted neutrophil before treatment.Then we divided the neutrophil into control group and treatment group and extracted healthy adults' peripheral venous blood to extracted neutrophil as observation group to culture in vitro at the time.Three groups all joined fetal bovine serum,and the treatment group joined extra emodin.Finally we used flow cytometry to detect the apoptosis situation of neutrophil after 24 hours.Results: 1,retrospective analysis:(1)Two groups difference of patients' age,sex,cause of general information had no statistical significance(P>0.05).Two groups have comparability.(2)The combination therapy with routine western medicine treatment and the Dachengqi decoction treatment helped to shorten the time significantly for the disappearance of Abdominal pain,distension and tenderness,the hospital stay and White blood cells,neutrophils,blood amylase,urinary amylase recovery(P<0.05).And reduced the incidence of complications(P<0.05),but the total effective rate difference had no statistical significance between the two groups(P>0.05).2,prospective study:(1)Two groups difference of patients' age,sex,cause of general information had no statistical significance(P>0.05).Two groups have comparability.(2)The combination therapy with routine western medicine treatment and the Dachengqi decoction treatment helped to shorten the time significantly for the remission of abdominal pain,the first defecation,the improvement of gastrointestinal function(P<0.05).Through comparing the changes of blood amylase and neutrophils apoptosis of two groups before treatment and after different time periods treatment,we found that the difference was no statistical significance in the two groups after 24 hours of treatment(P>0.05),but the difference was statistical significant in the two groups after 48 hours and 7 days of treatment(P<0.05).Compared two groups,the treatment group's the incidence of complications had decreased,but the difference was no statistically significant(P > 0.05),the total effective rate difference was no statistical significance(P>0.05).(3)Culture neutrophil in vitro,Emodin treatment group of neutrophil apoptosis rate was significantly increased(P<0.05).Conclusion: Routine western medicine treatment combining with the Dachengqi decoction treatment early can improve patients' clinical symptoms and signs in the early,shorten the hospital stay and reduce the incidence of complications observably,improve the effect of clinical treatment,show the advantage of combining traditional Chinese and western medicine treatment of SAP.It deserves clinical popularization and application.
