Integrative Medicine Clinical Observation Of Treatment Of Severe Acute Pancreatitis

Severe Acute Pancreatitis is a frequent acute abdomen and presents the feature such as acute onste, fast progress and a complex change of clinical pathology.It has SIRS, MODS in the early time and its fatality rate is high, so it threatens people's health seriously. The therapy of SAP has experienced the different evolution, on last century 80's ,it positions early time positive surgery and regularity pancreatic gland excision, but the result is that it cannot improve the prognosis of illness complication such as initial period shock, pancreatic gland widespread necrosis and organ failure. The following attempt for the individual conservative treatment can reduce the illness complication, but the fatality rate has no substantive improvement. Along with the development of intensive care, image technology and the treatment medicine, especially with the progress in the therapy and the surgery strategy, the fatality rate drops comparatively, but there is still 20-30%. In recent years, massive clinical research indicated that using the way of integrated traditional and western medicine to treat the SAP can obtain a better clinical curative effect, it also can obviously drop the illness complication and reduce the surgery and fatality rate of SAP, and the average hospitalized time becomes shorter.Aim: To observe the clinical curative effect of the therapy on SAP by integrated traditional and western medicine, for better using it in clinic, and providing new ideas for the research in the futureMethods: We collected 97 cases of AP admitted to DongZhiMen Hospital from year 1995 to year 2007, find out 42 cases of SAP from them to make a retrospective research. According to the different therapy we depart the group into conventional therapy group (control group) and integrated traditional and western medicine therapy group (therapy group), contrasting two groups'recovery of abdominal symptoms, signs, blood and urine amylase enzyme, and comparing the cure rate and the efficient rate.Results: (1) The recovery of abdominal symptoms, signs, blood and urine amylase enzyme of the therapy group is quickly than the control group;(2) Early cure rate and efficient rate of the therapy group is higher than the control group.Conclusion: To treat the SAP patient by integrated traditional and western medicine will reach a curative effect,and it can improve the early cure rate of the SAP. According to the dialectic of traditional Chinese medicine, the SAP will be divided into three phases: initial stage (systemic inflammatory response period), advanced stage (systemic infection period), recovery stage.In different stage, we used different therapeutical principle to guide the clinic therapy, embodied the advantage of integrated traditional and western medicine. During the research, we also found that the therapy of integrated traditional and western medicine could shorten the course of SAP; most of the patients may progress directly into the recovery phase without into the advanced phase, which is the criticality of the therapy of integrated traditional and western medicine.
