Objective:In this study,we measured the biochemical indexes of ACS patients,including Red blood cell distribution width variation coefficient RDW-CV,plasma fibrinogen(FIB)and apolipoprotein A-I(ApoA-I).A preliminary study of the relationship between blood markers in ACS patients and the correlation with Gensini scores reflecting the degree of stenosis of coronary artery disease.Methods:From September 2015 to September 2017,137 patients(male: 79 cases,female: 58 cases)was admitted to the Department of Cardiology of our hospital and underwent coronary angiography(Coronary arteriography CAG)in our hospital cardiac catheterization room.106 patients who were diagnosed as ACS with clinical data were used as experimental group.According to the diagnostic criteria of ACS,they were divided into unstable angina pectoris(1 group,n=34 case),non ST segment elevation myocardial infarction(2 group,n=35 case),ST segment elevation myocardial infarction(3 group,n=37 case).At the same time,31 patients who were diagnosed and excluded ACS were used as the control group,and the coronary lesions were estimated with the Gensini score.The baseline data of the ACS and the control groups were compared.At the same time,the data were compared between the 3 groups.At the same time,according to the RDW-CV four quantile stratification,the overall object Gensini score was compared and the Gensini score in the experimental group was compared.Based on bivariate correlation analysis,the correlation between ACS and Gensini score and risk factors was analyzed,and binomial Logistic regression for ACS and regression analysis for Gensini were performed.Results:1.Comparison of statistics between experimental group and control groupThe Fib of the patients in the experimental group was significantly higher than that in the control group(P=0.001).There was no significant difference in RDW,ApoA-I between the two groups(P<0.05).The prevalence of hypertension in the control group was higher than that in the experimental group(P<0.05),But the value of fasting plasma glucose(FPG)in experimental group was significantly higher than that of control group(P<0.05).There was no significant difference between the other general data(P>0.05);2.Comparison between the subgroups of experimental groups2.1 subgroup comparison of general data The age of the group 1 and the group 2 was higher than that in the group 3(P<0.05),The male prevalence rate in the group 3 was higher than that in the group 1(P<0.05),BMI was higher in group 1 than in group 3(adjusted after P=0.04),There was no difference in the prevalence of hypertension among the subgroups of the experimental group,SBP was significantly lower in group 3 than in other 2 groups(P<0.05),The score of Gensini in group 3 and group 2 was significantly higher than that in group 1(P<0.05),There was no significant difference in the prevalence of diabetes,DBP,HDL-C,LDL-C,TC and TG in the three groups(P>0.05);2.2 Comparison of subgroups of RDW-CV,FIB and ApoA-I There was no difference in FIB between the group 1 and the 2 group(P>0.05),The FIB and RDW-CV of the group 3 were significantly higher than that of the group 1(P<0.05),At the same time,the ApoA-I of group 1(P<0.05)was significantly higher than that of group 2 and 3 group(P<0.05);3.parameters comparison according to the level of RDW-CV four quantile level and correlation and regression analysis of ACS and Gensini scores3.1Median comparison of Gensini score after stratification of RDW-CV four quantilesThere was no significant change in the median of Gensini scores in the overall level of RDW-CV four quantile levels(P>0.05),The median of Gensini scores in the ACS group was different after stratification(P<0.05);3.2 Spearman rank correlation analysis with various factors and binomial Logistic regression analysis in ACSAccording to the Spearman rank correlation analysis,there was a positive correlation between sex,age,FIB,RDW-CV,hypertension and ACS in overall(P<0.05),BMI,SBP,ApoA-I have negative correlation(P<0.05).Binomial logistic regression analysis was used to analyze the independent positive correlation between hypertension,FIB and ACS(P<0.05);3.3 Spearman rank correlation analysis and linear regression analysis of Gensini score and various factors in the experimental groupIn the experimental group,the Gensini score was positively correlated with hypertension,FIB,and RDW-CV(P<0.05),There was a negative correlation with the ApoA-I(P<0.05).Linear regression showed that hypertension,FIB and RDW-CV were positively correlated with Gensini score,and ApoA-I was negatively correlated with its independence.Conclusion:1.hypertension and FIB are independent risk factors for ACS,Compared with the control group,FIB in the ACS patients were significantly higher than the control group.RDW-CV may not be an independent risk factor for ACS;2.in patients with ACS,hypertension,FIB,Apo A-I,and RDW-CV were independent predictors of Gensini score,With the increase of FIB and RDW-CV,the degree of coronary lesion was aggravated,At the same time,ApoA-I may be a protective factor for coronary artery disease.
