Objective: The red blood cell volume distribution width(RDW)is a part of the routine blood testing,reflecting the uneven size of circulating red blood cells,red blood cells reflect the high RDW value(RBC)of the large volume change.This parameter has been shown to predict cardiovascular disease such as heart failure,coronary artery disease stable and acute myocardial infarction(AMI),related indicators of morbidity and mortality in this study.Retrospective data analysis RDW in patients with acute coronary syndrome(ACS)patients and the correlation between the changes and the severity of coronary atherosclerosis,and analysis RDW and white blood cell count(WBC).A possible mechanism for the etiology of the disease is provided.Method:Methods a retrospective study was conducted in 216 patients with coronary heart disease who were admitted to the Department of Cardiology,Shaanxi People's Hospital from October 2015 to-2016 in October.1.According to the diagnosis of coronary angiography,the changes of serum myocardial enzymes and history were divided into ACS group(163 cases)and normal control group(n = 53).The clinical data were compared between the 2 groups.2.ACS group according to the different clinical types are divided into: unstable angina pectoris(UA)in 73 cases,non ST elevation myocardial infarction(NSTEAMI)in 24 cases,ST elevation myocardial infarction(STEAMI)in 66 cases.According to the number of coronary artery vascular lesions in the ACS group was divided into single vessel disease group,double vessel disease group and multi vessel disease group according to coronary angiography.According to the lesion vascular sites,number of vascular lesions,vascular stenosis degree counted Gensini score.To investigate whether there is a linear correlation between RDW,WBC and Gensini integration,and whether the occurrence of ACS or not is a dependent variable,and to analyze the risk factors of ACS by multivariate logistic regression analysis.Results: 1.General clinical data: ACS group in gender,smoking,history of hypertension,history of diabetes,serum creatinine was significantly higher than that of the control group,P<0.05;Based on the number of vascular lesions in the Sanya group compared with the analysis of variance,TG,total cholesterol(TC),low density lipoprotein(LDL-C),the difference was statistically significant.2.The RDW level of ACS group was significantly higher than that of control group(P<0.05).3.The number of RDW was increased with the increase of the number of vascular lesions in the subgroups of different vascular lesions.4.Comparison of RDW levels in different clinical types of Sanya group and control group,the results showed that: UA group,NSTEAMI group,STEAMI group,the three groups were compared with the control group,RDW value was significantly increased,the difference was significant,P<0.001.STEAMI group were compared with the UA group,the difference was significant,P value was P=0.006.There was no significant difference between UA group with STEAMI group and USTEAMI group in the mean of RDW.5.Correlation analysis between Gensini integral and RDW and WBC,the results showed that there was a significant positive correlation between RDW and WBC and Gensini integral,the correlation coefficients(r=0.501,P<0.001)and(r=0.379,P<0.001);RDW and WBC have positive correlation,the correlation coefficient(r= 0.361,P<0.001).6.Multivariate logistic regression analysis of risk factors of ACS showed that RDW was an independent risk factor for ACS,and high density lipoprotein cholesterol(HDL-C)was the protective factor.Conclusion:1.RDW levels in ACS group is significantly higher than the control group.2.The increase of RDW value was positively correlated with WBC,which indicated that the increase of RDW might be related to the chronic inflammatory reaction3.The increase of RDW value was positively correlated with the severity of coronary lesion(Gensini score),which indicated that the increase of RDW could reflect the severity of coronary artery lesion.4.Logistic regression analysis in the control of other factors,RDW increased to ACS alone HDL-C was elevated as independent protective factor.
