ObjectivesTo explore the effect of the self-management model based on “maltose” APP that was used in patients with type 2 diabetes in improving self-management capability,controling their own blood sugar,blood lipids and other clinical indicator,and improving their quality of life.Methods150 patients who met the inclusion criteria were randomly assigned into two groups with 75 cases in each group.The control group used telephone follow-up and hospital visits,while the experimental group used the “maltose” APP which was directed by “4C”continuous nursing theoretical framework for continuing care,plused the measures taken by the control group.In six months after the intervention,patients' related clinical indicators such as blood sugar,blood lipids and other clinical indicator,self management level,quality of life were measured.Summary of diabetes self care activities(SDSCA)and adjusted diabetes-specific quality of life scale(A-DQOL)were used respectively to evaluate self management level and quality of life.SPSS 21.0 was used and statistical methods including descriptive statistics,chi-square test and t test were used.ResultsThe two groups patients' general data,the blood sugar,blood lipids and other clinical indicator,self-management capability,quality of life showed no statistically significance(P>0.05),and the baseline data were comparable.The experimental group revealed statistically significance compared with the control group at the blood sugar,blood lipids,other clinical indicator,self-management capability and quality of life by intervention of continuing care model based on“maltose” APP(P<0.05).(1)The blood sugar,blood lipids,liver function and renal function: The experimen-tal group patients' fasting plasma glucose,hemoglobin A1 c,low density lipoprotein and high density lipoprotein were better than the control group and the difference shows statistical significance(P<0.05).The other index(including total cholesterol,triglyceride,alanine aminotransferase,aspartate aminotransferase,blood urea nitrogen and serum creatinine)showed no statistical significance(P>0.05).(2)After 6 months' intervention,self-management capability(dietary selfmanagement,exercise self-management,self-monitoring of blood glucose,foot care and medication compliance)in the experimental group patients were better than those in the control group,and the differences were statistically significant(P<0.05).(3)After 6 months' intervention,the experimental group patients showed statistical significance than the control group at adjusted diabetes-specific quality of life scale,psychology/mind,social relationship,influence of treatment and total scores(P<0.05).The experimental group was better than the control group.ConclusionsType 2 diabetes patients continuing care model based on “maltose” APP could improve the patients' related clinical indicators,their self-management capability on diabetes and quality of life.The continuing care model could be extended.
