Objective:This study investigates the current situation and related factors of diabetic patients’continuing care needs in communities, finds out the problems in this prospective, and provides guidelines and suggestions for clinical practice. This study could be serve as guidelines and references for the community nurses to offer needs oriented continuing care in the future.Methods:Random sampling method was used to select 200 diabetic patients as research objects from the Endocrinology Department of one hospital in Kunming from May to September in 2014 according to the criteria of inclusion and exclusion, by self-designed questionnaire. And SPSS 17.0 was utilized to do the statistical description and analysis.Results:1. Socio-demographic characteristics are described as follow. The patients are aged 23~88 (56.87±0.85), mainly aged 40~60 (47.5%), the vast majority of them are married (90%), most of them (59%) have the education background from senior high school or technical secondary school or lower, and 55% are retired.2. Situations related to daily life:the majority of their income is less than RMB3500 (56%); the main income is pension (55%); 58.5% of the patients have never been to community health service institutions; 75.5% of them are covered by the Urban Employee Medical Insurance.3. situations related to diseases:51% of the participants have got diabetes more than 5 years; 69% of the participants accepted the health education of diabetes; 27% of the patients chose the "oral medications and diet control and sports control" method.57.5% of the patients have got complications, with 35% diabetic retinopathy among them.56.5% of the patients accompanied with other kinds of chronic diseases, and hypertension is the most common one (43%). Family members who have diabetes accounted for 47%.4. patients’ attitude to community continuing care:84% of the participants hope to get community continuing care needs after discharge. Consulting the doctor become the most popular way to healthcare service (58.5%).5. the influential factors of diabetics’ community continuing care need after discharge:the study found that the following six factors, they are:patients’ attitudes, disease course, level of healthy education, family history, co-morbid diseases and health insurance types.Conclusions:the study found that following six kinds of diabetic patients need more community continuing care:patients without healthy education, patients without health insurance, patients with shorter disease course, patients without family history, patients combined with other chronic diseases, patients who hope to get community continuing care. Community healthcare team should provide corresponding nursing service for them according to current situation and influential factors. Thus the community resources can be utilized fully to control their diseases and improve their quality of life.
