Clinical Observation On Internal Ligation External Excision And Lateral Internal Sphincterotomy In The Treatment Of Circular Mixed Hemorrhoids

Objective:Thorough the clinical observation on the Internal ligation external excision and lateral internal sphincterotomy in the Treatment of circular mixed hemorrhoids。aims to summarize the best opplication of indications to use lateral interal Sphincterotomy. Methods: Thirty (30) patients with circular mixed hemorrhoids were included in this prospective tial.the patients were randomized into two equal groups of 15 patients.Patients from the control groups underwent internal ligation external excision. Patients from the therapy group quite apart from internal ligation external excision, underwent lateral internal sphincterotomy; Routine anorectal manometry were done for each patient before surgery.Parameters elicited in the study were the hospital stay, clinical curative effet,postoperative pain, urinary retention, edema,postoperative bleeding,fecal incontinence,anal stenosis. Results:There was no significant difference in the hospital stay (P=0.32), pain at 12 hours after surgery (P=0.163),urinary retention (P=0.717), postoperative bleeding (P=0.689),edema (P=0.472),the wexner incontinence scale (P=0.682) between the two groups;These differences were statistically significant in the postoperative pain in each of the two categories at the first bowel movement,one week after sugery and painkiller consumption;There was no difference in all the observed targets of the normal anal resting pressures patients in each of the two categories; These differences were statistically significant in the raised anal resting pressures patients of the two categories at pain of first detaecation (P=0.003), pain of one week after surgery (P=0.O11), edema (P=0.046) and anal stenosis (P=0.028); There was significant differences in the patients with incarceration of the two groups at pain of 12 hours after surgery (P=0.034),pain of first detaecation (P=0.025), pain of one week after surgery (P=0.034) and edema (P=0.025). Conclution:Internal ligation external excision and lateral internal sphincterotomy cures the circular mixed hemorrhoids off completely,and can relieve the Post-operative pain, Edema, avoid anal stenosis of the patients with the raised anal resting pressures or with incarceration.
