| Objective:1.To explore the correlation between hormone dependence dermatitis and demodex,and the clinical outcomes of cool blood detoxification formula,a traditional Chinese medicine,in suppression of demodex by measuring the number of facial demodex and quantity as well as scores of demodex colonization before and after treatment in patients with hormone dependence dermatitis(HDD).2.To observe clinical advantages and safety of TCM by clinical outcomes observation of cool blood detoxification formula in the treatment of HDD(hot and toxic accumulation syndrome).Methods:1.Sixty-four patients with HDD met inclusion and exclusion criteria were included in observation group,and 40 facial healthy patients without skin lesions were selected as control group.The number of demodex was measured in the same locations and areas by fat-scrapping method in the two groups,and scores was evaluated according to the Demodex Colonization Status Scale.2.A total of 90 patients with HDD(heat and toxic accumulation syndrome)met inclusion and exclusion criteria were randomly divided into two groups.The observation group(n=45)was given cool blood detoxification formula and Baixuan Xiatare Tablets in combination,while the control group applied hyaluronic acid gel dressing in combined with Baixuan Xiatare Tablets.Besides,external application of Tacrolimus Ointment was given to both groups for 4 weeks(1~2 weeks,twice daily;3~4 weeks,once daily,respectively).The Baixuan Xiatare Tablets was orally administrated at a dose of 0.9g,three times per day;cool blood detoxification formula and hyaluronic acid gel dressing,the external used drugs,were applied three times per week,once a day,respectively.The number and scores of demodex colonization were recorded after mite microscopy before treatment,and at 2,4 and 6 weeks after treatment.Clinical outcomes and adverse reactions of two groups were evaluated in two groups at 2,4 and 6 weeks after treatment.Results:1.Patients with HDD had higher positive rate of demodex,Colonization quantity and scores than the health controls(P<0.05)2.The observation group had significantly higher total response rate than the control group after 2-week treatment(19.05% vs.4.76%,P<0.05).The total response rates at 4,6 weeks were 69.05%,64.29% in the observation group and 85.71%,83.33%,in the control group,but no significant difference was seen in two groups(P>0.05).3.The scores of itching and burning symptoms in the observation group were lower than that in the control group after 2 weeks of treatment(P<0.05),but showed no significant difference at 4,6 weeks after treatment.The control group had lower pimple symptom scores and dry symptoms than the observation group after 2,4 and 6 weeks of treatment(P<0.01),but there was no between-group significant difference in the scores of erythema and telangiectasia after 2,4,and 6 weeks of treatment.4.The scores of demodex colonization quantity in the observation group were significantly lower than that in the control group(P<0.05).Conclusion:1.Patients with HDD have higher positive detection rate of demodex,number and scores of colonization of demodex than facial healthy controls,which further prove that hyper proliferation of demodex is correlated with hormone-dependent dermatitis.2.The observation group with cool blood detoxification formula,Baixuan Xiatare Tablets,Tacrolimus Ointment in combination and the control group with hyaluronic acid gel dressing,Baixuan Xiatare Tablets,Tacrolimus Ointment in combination have better and equivalent efficacy in treating HDD,but the clinical outcomes of observation group was better than that of the control group within 2 weeks.3.The observation group had better short-term clinical efficacy in the improvement of itching and burning sensation than the control group,while the control group had better clinical outcomes in the improvement of dryness,and the improvement of pimples in the observation group was better than the control group,but there was no significant between-group differences in improvement of erythema and telangiectasia in all stages of treatment.4.Cool blood detoxification formula has better efficacy in inhibiting the proliferation of demodex in the facial skin than hyaluronic acid gel dressing,and it is inhibitory for demodex.5.Cool blood detoxification formula,Baixuan Xiatare Tablets,Tacrolimus Ointment in combination,characterized by prominent inhibitory effect on demodex,simplicity,and low price,so it is worth of clinical application. |
