Objective: To investigate the difference of microsurgery in patients with simple sacral and nerve root sacral cysts and to compare the prognosis of two groups.Methods:The retrospective study of 34 cases of sacral canal cyst treated in our department from February 2012 to September 2017 was performed.According to the clinical symptoms,imaging features and intraoperative findings,the patients were divided into two groups: simple type and nerve root type.The difference of operation strategy was compared between the two groups.The follow-up cases were followed up and the IJOA score of was used to evaluate the prognosis.In this study,SPSS 22.0software was used for statistical analysis.Results: 34 patients were followed up for an average of 35.44 months,no case of recurrence.In this study,postoperative improvement was ≥ 25% in 33 patients,microsurgery was 97% effective,and 18(53%)achieved good standards(≥ 50%improvement).The preoperative IJOA score was 21.82 ± 1.71.The postoperative IJOA score was 25.00 ± 1.39.The statistical value of t = 19.778(p = 0.000)was statistically significant and statistically significant.The preoperative IJOA score of operation 1 was22.09 ± 1.69,and the postoperative IJOA score was 25.23 ± 1.31.The statistical value of t = 15.635(p = 0.000)was statistically significant and statistically significant.The preoperative IJOA score of operation 2 was 21.33 ± 1.72,and the postoperative IJOA score was 24.58 ± 1.51.The statistical value of t = 11.663(p = 0.000)was statistically significant significance.The overall improvement rate of 34 patients was44.95% ± 10.87%,and the improvement index was 3.18 ± 0.94.The improvement rate of patients with simple cyst was 45.97 ± 11.12% and the improvement index was3.14 ± 0.94.The improvement rate of patients with nerve cyst was 43.08 ± 10.60%and the improvement index was 3.25 ± 0.97.(Note: Operation type 1: All the cysts were removed under microscope,and the cyst and neck were ligated;Operation type 2:The partial resection of the nerve roots under microscope and the folding of nerve root sleeves were performed under the microscope)Conclusions: For the patients with symptomatic sacral cyst,microsurgical operation is recommended.For simple type sacral cyst patients underwent microscopic cyst resection and cyst neck ligation,for the patients with sacral canal cyst of nerve root type underwent partial resection of cyst under microscope and reconstruction of nerve root cuff with foldable nerve wall,this microsurgical strategy can significantly improve patient symptoms and benefit patients.
