Adipose tissue is currently considered as a hormonally active system in the control of metabolism and not only as a store of excess energy. It's found that adipocytes produce and secrete a variety of biologically active molecules, conceptualized as adipocytokines. Adiponectin is a recently discovered adipocytokine which is specifically expressed in white adipose tissue. Many studies show that it is found to be decreased in patients with obesity, type 2 diabetes, cardiovascular disease, and that hypoadiponectinemia is associated to ischemic heart disease.Adiponectin may have anti-atherogenic, anti-inflammatory and anti-neointimal proliferation properties. The objective of this study is to explore the predictive values of adiponectin for coronary artery disease and the relationship between inflammatory mediators and adiponectin. Methods All subjects were enrolled from inpatients who underwent coronary angiography. We studied total of 105 male patients. These included 80 patients with CAD,25 controls.Serum concentration of adiponectin and hs-CRP was assayed by RIA and rate nephelometry with the Beckman system.TNF-αand IL-6 were measured by ELISA.Meanwhile , plasma total cholesterol, triacylglycerol,smoking were measured and body mass index,body fat content were calculated. Data are presented as mean±SD.The significance of the differences between the groups were determined by the ANOVA. Linear regression analysis was used to evaluate the relationship between the variables.Results Serum adiponectin levels in the CAD patients were significantly lower than those in the control subjects (log adiponectin0.69±0.21vs 0.81±0.28,P<0.01). Serum concentrations of adiponectin were significantly decreased in acute coronary syndrome(log adiponectin: 0.64±0.18 mg/L), there was no difference in concentrations of adiponectin between stable angina pectoris(0.81±0.21 mg/L) and normal controls(0.81±0.28 mg/L). Serum adiponectin level was negatively correlated with BMI and body fat content(r=-0.28and -0.10,P<0.01).The inverse correlation among serum adiponectin,hs-CRP and TNF-αremained significant despite correction for obesity-associated (partial r=-0.42, P<0.01; partial r=-0.20,P<0.05). Conclusions We observed hypoadiponectinemia in coronary heart disease patients,especially in ACS group. Adiponectin reflects the stability of coronary artery stenosis which may predicts the occurrence and development of coronary artery disease.Low levels of serum adiponectin are associated with higher levels of hs-CRP and TNF-α. The level of adiponectin has the predictive value in the generation of atherosclerosis, and may be useful in evaluating prognosis of coronary artery disease.
