| Objective:By analyzing the difference of serum fibroblast growth factor 21(FGF21)levels between patients with coronary heart disease and non-coronary heart disease patients,and analyzing the relationship of FGF21 and the severity of coronary atherosclerosis,to evaluated the developent relationship of FGF21 and coronary heart disease.By analyzing the correlation between FGF21 and serum cholesterol ester transfer protein(CETP)and high-density lipoprotein(HDL-C)in patients with coronary heart disease is used to speculate whether FGF21 affects high-density lipoprotein by affecting CETP content,which maybe provide a new ideas for treatment and prevention CHD.Methods:214 patients who were hospitalized in the Department of Cardiology of the Second Clinical Hospital of Shanxi Medical University from December 2018 to June 2019 and had coronary angiography performed by two or more cardiac physicians were selected as the study.The subjects were selected between the age fluctuations and the age of(57.18±14.63),and were divided into two groups according to the results of coronary angiography: coronary heart disease group and non-coronary heart disease group,of which coronary heart disease group(n = 122)and non-coronary heart disease group(n = 92).Collect general information about all patients admitted for testing,including gender,age,smoking history,body mass index(BMI),presence of diabetes,triglycerides(TG),and total cholesterol(TC),High-density lipoprotein(HDL-C),low-density lipoprotein(LDL-C),and homocysteine(Hcy),calculate the Gensini score of each patient based on the results of the angiography,and collect the patient’s serum using enzyme-linked immunosorbent to measure the serum FGF21 and CETP levels.The data of each patient was analyzed using SPSS 20.0 statistical software,and then the serum FGF21 level and the incidence of coronary heart disease and the severity of its development were estimated(estimated by Gensini scoring system)And its relationship with CETP levels in the serum of patients with coronary heart disease.Results:1.The levels of TC,TG,and LDL-C in patients with coronary heart disease were significantly higher than those in the control group,and the differences were statistically significant(P <0.05),as follows: TC(3.83 ± 1.31 mmo /L,3.16 ± 1.73mmol/L),TG(3.64±3.23mmol/L,2.22±1.86mmol/L),LDL-C(3.79±1.05mmol/L,2.98± 1.11mmol/L).The level of HDL-C in patients with coronary heart disease was lower than them without coronary heart disease.HDL-C(0.80 ± 0.24mmol/L,1.03 ± 0.34mmol/L)and the difference was statistically significant(P <0.05).The other indicators were comparable,but no Statistical differences.2.The comparison between the two groups found that the serum levels of FGF21 and CETP in patients with coronary heart disease were higher than those in patients without coronary heart disease,of which FGF21(588.46 ± 147.51 pg / ml,449.25 ± 145.18 pg / ml),and CETP(73.45±15.73 ng /ml,63.50 ± 11.52 ng / ml)and the difference was statistically significant(P <0.05).3.After adjusting for factors such as age,gender,and smoking history,the presence or absence of coronary heart disease was used as the dependent variable,and HDL-C,LDL-C,FGF21 were used as independent variables.The result of multivariate logistic regression analysis showed that FGF21(OR = 1.005,P <0.05),LDL-C(OR = 1.833,P <0.05)increased levels were risk predictors of coronary heart disease,and HDL-C increased(OR = 0.142,P <0.05)are protective factors of coronary heart disease,and the ROC curve of FGF21 concentration and coronary heart disease shows that FGF21 has predictive value for the diagnosis of coronary heart disease,and the optimal cut-off value is 592.143(pg / ml).4.After adjusting factors such as gender,age,TC,TG,and LDL-C,the relationship between serum FGF21 level,CETP level,and HDL-C was compared using Pearson correlation analysis.The results showed that there was a significant negative correlation between HDL-C and FGF-21(r <0,P <0.05),while there was a significant positive correlation between CETP levels and FGF-21(r> 0,P <0.05),in the coronary heart disease group.5.In the coronary heart disease group,patients were divided into low group,middle group and high group according to Gensini score.The result of Spearman correlation analysis between FGF21 and it showed that FGF21 was positively correlated with the severity of coronary artery disease.Conclusion:1.The results showed that the serum FGF21 concentration in patients with coronary heart disease was higher than who in the control group,and the increased FGF21 concentration was a risk factor for coronary heart disease.The optimal threshold for FGF21 to predict the occurrence of coronary heart disease is 592.143(pg/ml).2.The severity of coronary artery disease in patients with coronary heart disease is positively correlated with the concentration of FGF21,so it is further proved that an increase in FGF21 concentration may aggravate the development of coronary heart disease.3.Correlation analysis shows that the concentration of FGF21 is positively correlated with the concentration of CETP and negatively correlated with the concentration of HDL-C.Studies have shown that CETP can hinder the maturation of HDL,so it is speculated that the increase in FGF21 concentration may affect the level of HDL-C by increasing CETP.
