Objective:To investigate the relationship between serum fibroblast growth factor 23(FGF23)and coronary heart disease in patients with coronary heart disease and healthy people,and to explore the relationship between FGF23 and coronary heart disease.Methods:From January 2018 to January 2019,83 cases of coronary artery disease diagnosed by cardiologists were selected as the experimental group,which were diagnosed as coronary artery disease by coronary angiography in the Department of Cardiology of Yanbian University affiliated Hospital.At the same time,82 healthy persons matched with sex and age in the physical examination center of our hospital were selected as the normal control group.According to the number of narrow vessels,patients with coronary heart disease were divided into two groups,one group had single vessel disease,the other group had multi-vessel disease,multi-vessel disease included two-vessel disease,three-vessel disease and stenosis involving the main left coronary artery.The coronary heart disease group was divided into simple coronary heart disease group,coronary heart disease complicated with hypertension group and coronary heart disease complicated with diabetes group according to the diagnosis results.The general data of all subjects were collected and the related indexes were measured.the changes of serum fibroblast growth factor 231evels in each group were compared and the correlation between them was analyzed.Results:1.Fibroblast growth factor 23,low-density lipoprotein cholesterol(LDL),cystatin C(Cysc),and blood glucose(GLU)in subjects with coronary heart disease compared with healthy controls and the level of hypersensitive Creactive protein(hsCRP)was significantly increased(P<0.05),while the levels of total cholesterol(CHO),high-density lipoprotein cholesterol(HDL)and calcium(CA)were significantly lower(P<0.05);The prevalence of hypertension and diabetes in the coronary heart disease group was higher than that in the healthy control group(P<0.05).There were no significant differences between the two groups in other variables(P>0.05)2.Comparison of subjects with or without coronary heart disease showed that serum FGF23 levels were significantly elevated in subjects with coronary heart disease(289.13± 103.15pg/ml and 161.15 ±63.86pg/ml,P<0.001).For patients without coronary heart disease,single vessel disease,and multivessel disease,serum FGF23 levels were(161.15±63.86pg/ml),(248.22±77.23pg/ml)and(311.16±109.13pg/ml).The results showed that the serum FGF23 level in the single-vessel disease group was significantly higher than that in the non-coronary heart disease group(P<0.001),and the serum FGF23 level in the multi-vessel disease group was significantly higher(P=0.011).Serum FGF23 levels and cumulative stenotic vessels showed a significant upward trend(P<0.001;Figurel).And the level of serum FGF23 was positively correlated with the number of coronary lesions(r=0.304,p<0.005)3.Compared with the control group,serum FGF23 levels were significantly higher in the patients with coronary heart disease alone,coronary heart disease with hypertension,and coronary heart disease with diabetes(P<0.001).However,there was no significant difference in serum FGF23 among the three groups of coronary heart disease(P>0.05).4.Correlation analysis showed that serum FGF23 was negatively correlated with high-density lipoprotein cholesterol(HDL)(r=-0.293,P<0.001),and negatively correlated with calcium(Ga)(r=-0.305,P<0.001),negatively correlated with bloodurea nitrogen(BUN)(r=-0.232,P=0.003),and positively correlated with hypersensitive C-reactive protein(hsCRP)(r=0.461,P<0.001).5.Logistic regression analysis of risk factors for coronary heart disease showed that HDL(OR=0.031,95%CI0.004-0.270,P=0.002)was a protective factor for coronary heart disease,while serum FGF23(OR=1.024,95%CI1.014-1.1034,P<0.001)is a risk factor for coronary heart disease.6.The area under the ROC curve of serum FGF23 for the diagnosis of coronary heart disease was 0.873(95%CI0.819-0.927.P<0.001),the optimal cut-off value was 220.06pg/ml,the sensitivity was 0.725,the specificity was 0.900,and serum FGF23 has a reference value for the diagnosis of coronary heart disease.Conclusion:1.The level of serum FGF23 in patients with coronary heart disease is significantly increased,and the level of FGF23 is positively correlated with the number of coronary artery lesions.The level of FGF23 can reflect the degree of coronary heart disease to a certain extent.2.FGF23 is an independent risk factor for coronary heart disease.Determination of serum FGF23 level is an objective basis for the diagnosis of coronary heart disease.
