Postpartum Depression(PPD)is a common mental health problem among women.It often manifests itself in emotional ups and downs,being very sensitive to the things around them,sometimes excited and indifferent.In addition,women with postpartum depression often feel fatigue,poor appetite,lack of interest,irritability,difficulty concentrating,insomnia,down,and cry,severe cases will also have a tendency to commit suicide,which can bring pain and serious negative effects to the mother and family.According to the report,the prevalence of postpartum depression in Europe is 7.73%,and the prevalence rate in Asia is 3.5-63.3%.The incidence of depression was 10.08%in Shanghai and 27.37%in Guangzhou,and it was a rising trend,which attracted the attention of scholars.Recently,there are more and more studies on the risk factors of postpartum depression in social psychology and physiology.The result showed that during pregnancy and the whole process of perinatal production,the pregnant women have been in a strong and persistent state of stress,and this long-term stress has an effect on their psychology.Sleep,life events,social support and interpersonal relationship are all important factors affecting the occurrence,development and reintegration of postpartum depression.Considering the postpartum breast-feeding,as far as possible,do not take the needs of drug intervention,how in many social psychological factors,find the problem targets,optimize the intervention strategy,is a hot and difficult in the field of clinical nursing psychology study.Taking into account the actual need to avoid drug intervention as much as possible.How to find the problem target and optimize the intervention strategy in many social psychological factors are the hot and difficult point in the field of clinical nursing psychology.Objective:To analyze the factors affecting postpartum depression from the perspective of social psychology,optimize the intervention measures and evaluate the effect of intervention.Methods:817 cases of perinatal women were selected as the research object in a cardiac care unit of a Grade three specialist hospital.The questionnaire survey was respectively carried out in the prenatal period(more than 37 weeks of pregnancy)and postpartum(about 42 days after delivery),using Spearmen correlation analysis,multiple linear regression analysis method to analyze their relationship,the relation between mother-in-law and daughter-in-law,postpartum taboo,postpartum sleep problems and the factors such as social support and life events;According to the results of questionnaire survey,by focusing on family relations,and the use of Edmund&Heidegger's view of qualitative research,the author conducted a semi-structured interview with 14women with post-natal depression and made recordings and transcripts.Finally,Colaizzi's analysis program is used for analysis.The author searched the Randomized Controlled Trials of interpersonal psychotherapy for patients with postpartum depression(RCT)in PubMed(MEDLINE),EMbase,EBSCO,Chinese Biomedical Literature Database,China Knowledge Network,Wanfang Database,and Weipu Database,among which the required experiments were incorporated and meta-analysis was carried out.A total of27 women who were screened for postpartum depression were selected from the postpartum clinic of a gynecology department,and they were divided into the intervention group(13 persons)and the control group(14 persons)with the random number method.The author gives the control group the routine interpersonal psychological intervention,and gives the intervention group the interpersonal psychological intervention which is oriented by the family relationship.The author used the Edinburgh Postnatal Depression Scale(EPDS)to evaluate the Depression in the pre-intervention,post-intervention and 12-week follow-up.The results of this optimization measure were verified by using the statistical method of repeated measurement variance analysis and independent sample test.Result:1 The relationship between mother-in-law and daughter-in-law,relationship between husband and wife,social support,sleep,life events,antenatal depression associated with postpartum depression significantly(P<0.01),and into the multivariate regression model(P<0.05),the above factors can explain the variation of postpartum depression by 41%(P<0.01).Path analysis results showed that the relationship,old woman daughter-in-law relation can be applied to social support,sleep the two intermediary variables,in turn,affects postpartum depression(The model fitted well:?~2=11.9,df=4,?~2/df<3,CFI=0.99>0.9,TLI=0.964>0.9,RMSEA=0.049<0.05,P<0.01).The results indicated that the main factors influencing postpartum depression include the relationship between husband and wife,the relationship between mother-in-law and daughter-in-law,and the related life events,sleep and social support.2 The results of qualitative research further indicate that the relationship between mother-in-law and daughter-in-law,the relationship between husband and wife and the relationship between mother and child are the main themes of postpartum depression.The results indicated that family relationship was the main influencing factor of postpartum depression.3 10 randomized controlled trials of meta analysis showed that,with the Beck Depression Inventory(BDI)and Edinburgh postnatal Depression scale as the evaluation index,interpersonal psychotherapy have intervention effect on postpartum Depression(P<0.01);The results of sensitivity analysis and combined effect showed that interpersonal psychotherapy also had an intervention effect on postpartum depression(P<0.01),and both were stable and reliable.The results suggest that the psychological intervention strategy based on interpersonal relationship is effective.4 The EPDS scores of the two groups were both decreased and remained at normal level(<10 points)after 8 weeks of intervention and 4 weeks after intervention.The results of variance analysis showed that the comparison effect between the two groups was significant(P<0.05).The independent sample t test results showed that the difference between the EPDS score(baseline)and the intervention after 8 weeks was significantly higher in group B than in group A(P<0.05).In the four weeks after the intervention,the difference of group B was significantly higher than that of group A(P=0.05,because the sample size was small,and the effect quantity(Cohen's d)was greater than 0.68,and the difference was still significant).Twooptimizationresultsindicate,interpersonalpsychological intervention methods have significant interference effects,which focus on the optimization of family relationships than simple interpersonal psychological intervention method to shorten intervention when long optimization method is better.Conclusion:With relationship between mother-in-law and daughter-in-law,the relationship between husband and wife,maternal and child relationship as the theme of family relationships are the main influence factors of postpartum depression,two optimization of interpersonal psychological intervention can effectively reduce postpartum depression,and focused on the optimization of family relationships of interpersonal psychological intervention strategy is better than pure shorten intervention when long intervention effect,meaning more promotion.
