Hengshui City ICU Deep Venipuncture Investigation And Analysis Of Related Complications

Objective: Deep understanding of hengshui city intensive care unit(ICU)static catheter related complications,explore effective measures for decreasing the occurrence of it.Methods: Selection in January 2017-December 2017,hengshui city hospitals,a total of 1978 cases of ICU patients with percutaneous deep venipuncture,depending on the catheter way divided into subclavian catheter group 889 cases,620 cases of internal jugular vein catheter group and 469 cases of femoral vein catheter group,analysis of three groups of patients with catheter related complications,and a success rate of puncture,catheter indwelling time and comfort with pipe.Results: 1978 cases of patients,a total of catheter related complications including 50 cases with abnormal bleeding puncture point,puncture of artery 34 cases,catheter obstruction 24 cases,catheter related infections of 17 cases,subcutaneous emphysema,5 cases,5 cases with arrhythmia and pneumothorax in 2 cases,the incidence of complications was 6.93%,including subclavian catheter were kept complication rate was 8.10%(72/889),internal jugular vein catheter group,the incidence of catheter related complications was 6.29%(39/620),femoral vein catheter group,the incidence of catheter related complications was 5.54%(26/469).The puncture point incidence of abnormal bleeding 3 groups compare differences had no statistical significance(P>0.05);Puncture of artery,catheter obstruction,arrhythmia,the incidence of subcutaneous emphysema,pneumothorax subclavian catheter group is higher than the internal jugular vein catheter and femoral venous catheter group,puncture of internal jugular vein catheter group arteries,catheter obstruction,the incidence of arrhythmia is higher than the femoral vein catheter group,difference has statistical significance(P<0.01);The incidence of catheter related infections femoral vein catheter group > subclavian catheter group internal jugular vein group,differences were statistically significant(P<0.01).A success rate of puncture femoral vein catheter group is higher than subclavian catheter group and internal jugular vein catheter group,the difference was statistically significant(P<0.05);Longest time subclavian catheter indwelling catheter group,with tube supreme comfort subclavian catheter group,differences were statistically significant(P<0.05 or P<0.01).Three groups of patients after catheter related complications,all in a timely manner to give corresponding treatment,the symptoms are different degree of improvement,not have serious consequences.Conclusions: According to actual situation to choose the most suitable catheter patients way,grasp skilled each tube piercing technology,familiar with the local anatomy of puncture,and strict adherence to aseptic operation specification,deep venipuncture related complications is to reduce the ICU main measures.
