Objective:To determine the influencing factors of the effecacy of the catheter-directed thrombolysis(CDT)in patients with acute deep vein thrombosis(DVT)of lower extremity by comparing the clinical data between the ineffective group and the effective group after the catheter-directed thrombolysis.Methods:The clinical data of DVT patients underwent CDT were retrospectively analyzed in Department of Interventional Vascular Surgery,Affiliated Hospital of Xiangnan University from October 2016 to October2019.Demographic information of the included patients with DVT including gender,age,overweight,course of disease,limb pain symptoms,smoking history,drinking history,the related complications of iliac vein compression syndrome,hypertension,diabetes,injured fracture in recent 1month,stroke history,intravenous drug addication,history of malignant tumors,and the laboratory tests of platelet count,levels of fibrinogen,triglyceride,total cholesterol,low-density lipoprotein,and D-dimers were collected as influence factors to determine thrombolytic efficacy of CTD treatment.The clinical data were statistically analyzed by chi-square test,Fisher exact probability method and multivariate logistic regression.Results:A total of 42 patients were enrolled in this study,which had been identified into two groups including 18 patients in the ineffective group and 24 patients in the effective groupaccording to the efficacy evaluation after CDT treatment.There were statistically significant differences between the two groups in iliac vein compression syndrome,smoking history,age?60 years,fibrinogen>4g/L,and platelet>300×10~9/L(P<0.05).Multivariate logistic regression analysis was performed for the influence factors screened by univariate analysis(P<0.05),and the results showed that Iliac vein compression syndrome(odds ratio=10.815,95%CI:2.014-58.064;P=0.005),fibrinogen>4g/L(odds ratio=8.492,95%CI:2.368-30.448;P=0.001),and age?60 years(odds ratio=4.084,4.084,95%CI:1.207-13.818;P=0.024)were the top three influencing factors for catheter-directed thrombolysis in patients with acute deep vein thrombosis of lower extremities.Conclusion:Iliac vein compression syndrome,fibrinogen>4g/L,and age?60 years are the influencing factors for catheter-directed thrombolysis in patients with acute deep vein thrombosis of lower extremities.
