Objectives 1 Analysis of head and neck artery stenosis and plaque distribution in patients with acute cerebral Infarction.2 To explore the risk factors of acute cerebral infarction.3 To discuss the neurologic impairment in patients with acute cerebral infarction?to investigate the relationship between the NIHSS score with region of head and neck artery lesion and the NIHSS score's value on in patients with acute cerebral infarction.Methods From November 2015 to November 2016,198 patients with acute cerebral infarction in our department of neurology were selected as the study subjects,including 112 male patients and 86 female patients,all of them underwent head and neck CTA examination.Calculated the vascular stenosis rate of head and neck lesions,and analyzed the shape,distribution and plaque characteristics of head and neck artery plaques.Recorded the patient's gender,age,NIHSS,and inquired the detailed information about drinking,smoking and hypertension.The fasting blood glucose,total cholesterol(TC),high or low-density lipoprotein,and triglyceride(TG)were determined.SPSS17.0 statistical software was used to process all the data.Results 1 Stenosis of the head and neck,In this study,there were 1022 stenosis,and the bifurcation of carotid arterywas the most.2 Head and neck artery plaque,there were 539 of soft plaque,located at middle cerebral;258 mixed plaque,mainly in the common carotid artery;hard plaque was 225,followed by the distribution of artery in intracranial vertebral artery.3 Relationship between gender and the degree of carotid artery stenosis and the nature of plaques,the proportion of moderate and severe stenosis of head and neck vessels in males is higher than that in females,and mild stenosis and hard plaque is lower than that in females,P<0.05.4 The relationship between age and the degree of head and neck artery stenosis and the nature of plaques,the ratio of severe stenosis and occlusion of head and neck arteries in patients younger than 60 years is lower than that in patients older than 60 years old,P<0.05.5 Risk factors related to head and neck artery lesion,the group with vascular stenosisin drinking and smoking,hypertension,diabetes mellitus and hyperlipidemia were significantly higher than that of without vascular stenosis group,P < 0.05,drinking,smoking,hypertension,that diabetes and hyperlipidemia were risk factors of vascular stenosis.4 The relationship between head and neck artery stenosis and NIHSS,and the value of NIHSS in patients with acute cerebral infarction:(1)The NIHSS score in group with vascular stenosis was 12.15±0.87,the group without artery stenosis was 6.03±0.87,the NIHSS score in the stenosis group was significantly higher than the non stenosis group,there were significant differences between the two groups,P<0.05,neurological function in patients with acute cerebral infarction vascular stenosis is more serious injury.(2)The NIHSS score in the group with intracranial artery stenosis was 11.2±8.4,the group with extracranial artery stenosis was 14.6±7.5,and intracian or extracranial artery stenosis was 23.1±9.8,P<0.05,it showed that the NIHSS score has a certain predictability to vascular stricture in patients with acute cerebral infarction.(3)In the group that the NIHSS score was 7 or less,there were 74 cases of patients with good prognosis with acute cerebral infarction,and 31 patients with poor prognosis;In the group that the NIHSS score more than 7,there were 15 patients with good prognosis of patients,78 patients with poor prognosis,the former group with good prognosis was 4.38 times the later group,the difference was statistically significant,P<0.05,showed that the NIHSS score on the prognosis of patients with acute cerebral infarction have predictive value.Conclusions 1 The CT Angiography can accurately determine whether there is stenosis or not and show the distribution characteristics of stenotic vessels clearly in patients with acute cerebral infarction.2 hypertension,diabetes mellitus,hyperlipidemia,Heavy drinking and smoking are the risk factors of acute cerebral infarction.3 The stenosis of the head and neck can aggravate the degree of nerve function defect in the patients with acute cerebral infarction,and the NIHSS score has a certain predictive value for the position and prognosis of the stenosis of the blood vessels in the patients with acute cerebral infarction.
