Objective: To analyze the correlation between blood pressure and progressive ischemic stroke,and provide guidance for predicting and treating progressive ischemic stroke.Methods: The clinical data of 140 patients with acute cerebral infarction were studied in the Department of Neurology of Minda Hospital of Hubei Minzu University,from September 2017 to January 2020.All patients were divided into progressive group and non-progressive group(control group)according to whether the symptoms of neurological impairment were aggravated after cerebral infarction(increased NIHSS score ?2 points).general data(including gender,age,past medical history,SBP,DBP,NIHSS score on admission,etc.),ambulatory blood pressure monitoring index(24HSBP,24 HDBP,24h MBP,24 h SSD,24 h DSD),and other results(CT/MR,carotid ultrasound,etc.)were analyzed in both groups.Using SPSS 20.0 software to process the data by statistical method,when the single factor was compared between groups: the metrological data who accorded with the normal distribution were used t test,and the mean ± standard deviation(X ± S)was described;the non-conformance was described by the median or quartile spacing by using the rank sum test in the nonparametric test.The counting data is represented by the number of cases or percentage,using the chi-square test or the rank sum test in the nonparametric test or Fisher precision test.multivariate analysis using logistic regression analysis.P <0.05 was statistically significant.Results: 1.A total of 140 patients with acute cerebral infarction were included,and 36 patients' illness developed,accounting for 25.71%.2.The 140 ACI patients were divided into the group with history of hypertension and the group without hypertension: the index of SBP?DBP of ACI patients with hypertension was significantly higher than that of the group without hypertension,and the difference was statistically significant.3.Single factor comparison: The medical history of hypertension,SBP,DBP,24 h SBP,24 h SSD,24 h DSD and the NIHSS score of the progressive group and non-progressive group were statistically significant.4.logistic multifactorial comparison: 24 h SSD(X1),and NIHSS scores(X2)of the two groups were statistically significant(P=0.007,OR=1.108;P=0.003,OR=1.216).Logit(P)=-3.460+0.103X1+0.195X2.Conclusion: 1.In this study,the incidence of PIS is 25.71 per cent.2.For patients with ACI,history of hypertension,blood pressure at admission(including systolic blood pressure,diastolic blood pressure),NIHSS score at admission,and 24 h SSD?24h DSD?24h SBP were all influencing factors for the progression of cerebral infarction.3.The higher the value of 24 h SSD and the NIHSS score at admission are the independent high risk factors for progressive cerebral infarction.4.There is a clear correlation between blood pressure variability and progression after acute cerebral infraction.
