Clinical Study On Guipi Decoction Combined With Western Medicine In The Treatment Of Low Risk MDS-RA In Deficiency Of Both Qi And Blood

Objective:The objective of this study was to evaluate the clinical efficacy and safety of Guipi Decoction combined with conventional Western medicine in the treatment of patients with refractory anemia of Low-Risk myelody-splastic syndrome in deficiency of both qi and blood,in order to find new treatment ideas and effective treatment methods for the patients.Methods:All the 60 patients who meet the requirements in this study were selected from the inpatients and outpatients of the Hematology Division of Shanxi Hospital of Integrated Traditional and Western Medicine from June 2015 to January 2017.All patients were randomly divided into two groups before treatment,in which 30 cases were treated with routine treatment,as the control group;another 30 cases were treated with Guipi Decoction combination with conventional medical treatment,as the observation group(treatment group).As 3 cases were dropped in the process,eventually 28 patients in the treatment group and 29 patients in the control group underwent the efficacy observation and safety evaluation.Clinical observation lasted for 3 months.Eventually,we observed and recorded the clinical symptoms,peripheral hemogram,T lymphocyte subsets,bone marrow and blood transfusion before and after treatment in both groups.SPSS20.0 software was used for Statistical analysis.The statistical results were statistically significant when P<0.05.Results:1.After 3 months of treatment,there was 6 with basic relief,9 with partial relief,5 with improvement and 9 without response in the control group.The overall effective rate was 69.0%.Relatively,there was 16 with basic relief,9 with partial relief,2with improvement and 1 without response in the treatment group.Therefore the overall effective rate was 96.4%,which was significantly different from that of the control group(P<0.05).2.After treatment,the peripheral hemoglobin count of the patients in the treatment group increased significantly.The difference was statistically significant.Compared with those in the control group,the difference was statistically significant(P<0.05).3.After treatment,the levels of CD4,CD4/CD8 in the treatment group were increased(P<0.05)and the CD8 decreased(P<0.05).The difference was statistically significant(P<0.05).4.The changes of bone marrow hyperplasia were compared between the two groups before and after treatment.The difference was not statistically significant(P>0.05).Compared with before treatment,the treatment group showed significant reduction in morbid hematopoiesis of bone marrow erythroid cells,the difference was statistically significant(P<0.05).Compared with the control group,the difference was not statistically significant(P>0.05).After treatment,the number of primitive cells in bone marrow in the treatment group was decreased,and there was no significant difference between the treatment group and the control group(P>0.05).5.Compared the blood transfusion of the two groups after treatment,the amount of blood transfusion in the treatment group was significantly lower than that in the control group(P<0.05).6.After treatment,the total score of clinical symptoms of the two groups were lower than before treatment,the difference was statistically significant(P<0.05);the total score of clinical symptoms in the treatment group was significantly lower than that in the control group(P<0.05).Comparing TCM syndrome curative effect of the two groups,9 cases were cured in the control group,8 cases were effective,the total efficiency was 58.6%;1cases were cured in the treatment group,18 cases were cured,6 cases were effective,the total efficiency was 89.3%;Comparing the total efficiency of the two groups,there was significant difference(P<0.05).7.Safety Evaluation: During the course of treatment,both groups had one patient who suffered from mild liver damage with transaminase slightly increasing,which was considered casued by the side effects of stanozolol and disappeared after symptomatic treatment.Comparing the urine routine,stool routine,renal function,electrocardiogram of the two groups before and after treatment,there was no significant difference(P>0.05).Conclusion:It proved that Guipi Decoction Combined with western medicine has definite clinical effect in the treatment on MDS-RA(low risk)in deficiency of both qi and blood.It not only can reduce the clinical symptoms,improve patients' peripheral blood,T lymphocyte subsets and bone marrow,but also reduce blood transfusion dependence,improve the quality of life of patients.Therefore,it is worth further exploring.
