Background:Hypertension is a multifactorial disease.It is now agreed that hypertension is a disease caused by both genetic and environmental factors.However,both domestic and foreign,the existing treatment effect and management mode of hypertension are not satisfactory.The advent of the Internet era provides a new way of thinking for the management of hypertension.Hypertension is 200 million patients in our country.The chronic disease management system for hypertension has not been established.In order to reduce the incidence of cardiovascular disease and the burden of disease,the health China has put forward the requirement to promote the Internet to medical services.Objective:Through archiving the hypertensive patients in the Department of Cardiology of the Affiliated Hospital of Yangzhou University,we know the pathogenic factors of hypertension in this area,such as healthy living habits and medication compliance,and then make a preliminary study on the researchers through a new type of hypertension management model designed by ourselves to achieve preliminary through Internet technology.Objective to explore the management mode of chronic diseases such as hypertension,and to optimize the management service flow of chronic diseases to realize the continuous,full and active management mode of chronic disease management.Method:1.The exploration of the risk factors of hypertension:included in the 671 outpatient outpatients of the Department of Cardiology of the Affiliated Hospital of Yangzhou University from March 2017 to September 2017.Through the inquiry and questionnaire survey,the related data were collected and included in the hypertension and non hypertensive patients,and the high blood in Yangzhou area was found through data analysis.Risk factors for pressing pathogenicity.2.The construction of a new model of hypertension management:through theoretical study and reference to advanced experience at home and abroad,a management platform with different management modules is designed and developed by the representative of hypertension medicine.A new chronic disease management model based on Internet technology for hypertension intervention is preliminarily constructed to ensure the success of the research.Shi.3.Intervention evaluation:the subjects were intervened through the established Internet intervention platform.After 3 months of intervention,the related indexes of the patients were changed before and after the intervention.The management closely monitors the dynamic changes of the blood pressure of the patients through the management platform.If there is any abnormality,communicate with the patients in time and communicate with the patients.At the end of the study,the data and blood biochemical data after the intervention were obtained by questionnaire,and then the control of hypertension was analyzed by statistical software to evaluate the effectiveness of the new management system.Result:1.A total of 152 cases of hypertension patients were included in this study.The awareness rate of hypertension knowledge and the standard rate of hypertension were significantly improved after systematic intensive intervention.2.After adjustment,the lifestyle of the patients was adjusted positively,and the medication compliance of the patients was significantly improved.3.The average systolic pressure of 24 hours and the average diastolic pressure of 24 hours decreased significantly after the intervention of the contrast management system,and the mean blood pressure decreased to the normal level,and the difference was statistically significant(P<0.05).The coefficient of variation of systolic blood pressure at 24 hours before and after management was 0.13 + 0.03mmHg and 0.11 + 0.02mmHg,respectively,and the difference was statistically significant(P<0.5).Conclusion:The study explored the main risk factors of hypertension in Yangzhou,and provided new ideas for the management of blood pressure in hypertensive patients and extended the service to the family.
