ObjectiveTo investigate the types of self-management behavior of pregnant women with gestational hypertension(GH)and analyze the factors affecting the self-management behavior of pregnant women.To explore the application value of an internet-based multidisciplinary cooperative management model of the blood pressure control level,self-management behavior,disease cognition level,and maternal and infant adverse pregnancy outcomes in different types of pregnant women with hypertension.MethodsThis study was divided into two parts.1.Investigation and analysis of self-management types and influencing factors of pregnant women with GH: The convenience sampling method is used to select 175 pregnant women with GH who meet the requirements of research in Zhenjiang 2 third Class Hospitals for investigation and analysis of self-management behavior.The survey tool used is a self-made general status questionnaire,hypertension self-management behavior scale,the self-efficacy scale,GH knowledge scale.2.Based on the above cluster analysis of self-management behaviors,the multidisciplinary management intervention for pregnant women with GH on the internet was studied.The pregnant women with GH treated in a third Class Hospital in Zhenjiang City were selected as the research objects,and 50 pregnant women with GH included from February to August 2018 were selected as the control group,and the routine management mode of high-risk pregnancy outpatient service was adopted.In addition,50 pregnant women included from September 2018 to March 2019 as the intervention group.According to the level of pregnant women’s self-management,the pregnant women in the intervention group were classified and managed by establishing a multidisciplinary cooperative management team and an internet-based family management system for gestational hypertension.After the intervention,the differences in blood pressure control level,blood pressure compliance rate,self-management behavior,hypertension knowledge,and the incidence of maternal and infant adverse pregnancy outcomes in the perinatal period between the two groups were compared.Results1.The results of cluster analysis showed that 175 GH pregnant women could be characterized into 3 types: Type I pregnant women with GH have good self-management ability and the score of self-management behavior was(152.00±10.42 points).Type II pregnant women with GH have medium type self-management ability,and the score of self-management behavior was(105.84± 9.20).Type III pregnant women with GH have low self-control ability and the score of self-management behavior was(68.60± 9.92).2.Comparing the general data,there were statistical differences in the aspects of educational level,family economic income,self-efficacy,and gestational hypertension knowledge across the 3 types of pregnant women with GH(P < 0.05).Multiple stepwise regression analysis showed that education level and self-efficacy were significant factors affecting the self-management behavior of pregnant women with GH.3.The intervention results of multidisciplinary cooperative management showed that there were statistically differences in the aspects of blood pressure control level,blood pressure compliance rate,self-management behavior,hypertension knowledge,delivery pregnancy week,postpartum hemorrhage,neonatal weight(P < 0.05),and there were no statistically differences in the aspects of preeclampsia,cesarean section,intrauterine distress,preterm birth,neonatal asphyxia,low birth weight(P>0.05).Conclusion1.The self-management behavior of pregnant women with GH is generally at a medium level or slightly lower,and the self-management level of different pregnant women with GH varies greatly.Health professionals should give individualized care according to different types of pregnant women with GH,which can effectively improve the self-management ability of them.2.The self-management behavior of pregnant women with GH is affected by a variety of factors,and education level and self-efficacy are the main factors.3.The management model of the multidisciplinary cooperation on the internet can improve the blood pressure control rate,hypertension knowledge level,and self-management behavior of pregnant women with GH,and reduce the incidence of adverse pregnancy outcomes in the perinatal period,which is worthy of further clinical promotion and application.4.Classified management of pregnant women with GH can effectively improve the intervention effect of an internet-based multidisciplinary cooperative management model.
