Management Service Model Of Hypertension Based On Internet

Background : With the development of society and economy,the incidence of cardiovascular disease increased every year and hypertension is the most important risk factor for cardiovascular disease.In recent years,through the long-term management of hypertension,comprehensive prevention and treatment of hypertension at home and abroad have made great progress,significantly reducing the incidence of cardiovascular disease and disease burden.However,there are still some problems in the comprehensive prevention and treatment system of hypertension in China,such as lack of consecutive and professional management and so on.In addition,the current service model of hypertension management cannot meet the growing health needs of people.At the same time,with the progress and development of information technology,the state attaches great importance to the national health work,which brings new opportunities for the innovation and development of service model of hypertension management.Objective : To explore the operation mechanism of hypertension management based on Internet,optimize the hypertension management service process and establish the new integrated health management system based on steady state of health for individuals and groups to get active,continuous and whole health management.Methods:Firstly,the theoretical research was made to system review the development and present situation of domestic and international health management model and analyze the problems and opportunities of the health management model in our country,the framework of new model of hypertension management in line with national conditions was also explored.Then based on the theoretical research on the new model of hypertension management,achieve the construction of a health management platform with modern information technology.Finally carry out an empirical study on the new model of management for patients with hypertension to evaluate the effectiveness of the management.Results: In this study,with the combination of the Internet and hypertension management and the use of mobile Internet,Internet of Things,cloud computing and other information technology,an innovation theory of hypertension management model based on Internet was put forward and the health management service platform was developed through the demand analysis and the design of system function module.The new model provides professional,timely and humanity closed-loop service such as case management,condition assessment,health plan,family supervision,intervention and appointment for the individual or groups through enrollment,archiving,assessment,planning,intervention and phased assessment.In the study of 110 cases of hypertension with application of health management service model showed that after 12 weeks of follow-up,the decrease of systolic blood pressure in health management group was more obvious compared with the conventional management group(P <0.05).The control rate of blood pressure in health management group(64.3%)was significantly higher than that in control group(33.3%),the difference was statistically significant(P <0.05).Conclusions: Through the construction and application demonstration of service model of hypertension management based on Internet in which the service will be extended to families and communities,the effectiveness and development prospects for the of new service model of hypertension management was initially confirmed.It's important and meaningful to reduce social medical expenses.
