Object This article retrospectively analyzed the clinical data of 1107 patients wi th lung cancer in our hospital in recent years to understand the clinical character istics and changes of lung cancer in recent years,and to strengthen the understa nding of the clinical characteristics of lung cancer cases.Methods Collected 1107 new cases of lung cancer admitted to the Department of Respiratory Medicine of the First Affiliated Hospital of Anhui Medical Univer sity from January 2013 to June 2017.The patient's gender,age,smoking history,pathological type,clinical symptoms,chest CT manifestations,and Bronchoscopy performance,diagnostic method and staging were analyzed and the results were di scussed.Results 1.In this group of data,the most common pathological type was adenocar cinoma(41.55%),followed by squamous cell carcinoma(28.82%)and small cell carcinoma(16.53%).There were significantly more men than women,with a sex ratio of 2.91 to 1,the largest difference between the sexes was squamous cell carcinoma,with a ratio of 13.5 to 1,and the smallest difference was adenocarci noma,with a ratio of 1.21 to 1.Among different genders,there was a differenc e in pathological type distribution,and the majority of men were squamous cell carcinoma,followed by adenocarcinoma,small cell carcinoma,and women are m ainly adenocarcinoma.2.The age group of high incidence of lung cancer patients in this group is 60-79 years old,accounting for 64.68% of the total number of lung cancer,followed by 40-59 years old,accounting for 30.44%.The high incidence of lung cancer h as an increasing trend compared with the past.The pathological types of all age s were common with adenocarcinoma.3.The smoking status of different pathological types in male patients was differen t.The proportion of smoking in squamous cell carcinoma and small cell carcino ma was 92.93% and 88.68% respectively,which was higher than that of 74.90% in adenocarcinoma patients.4.The clinical manifestations of lung cancer are diverse and lack of specificity.T hereinto,In addition to common main symptoms cough and sputum,comparison between different pathological types,adenocarcinoma is easy to appear chest distr ess and shortness of breath(48.26%),and squamous cell carcinoma is prone to bl ood in the sputum or hemoptysis(43.26%).Chest CT is mainly manifested by pu lmonary mass(86.70%),and other common manifestations include pleural effusion(33.88%),obstructive pneumonia(21.13%),multiple nodules(14.94%),obstructive a telectasis(13.48%).Comparison between different pathological types,the proportio n of pleural effusion and multiple nodules in adenocarcinoma is higher than that of squamous cell carcinoma,small cell carcinoma,while the proportion of obstr uctive pneumonia and obstructive atelectasis in squamous cell carcinoma and sma ll cell carcinoma is higher than that of adenocarcinoma.Bronchoscopy showed lu minal growth type more common(51.38%),followed by infiltrating type(34.31%),squamous cell carcinoma and small cell carcinoma are mainly luminal growth ty pe(70.43%,56.30%),and adenocarcinoma mainly infiltrating type(50.85%).5.The most common definitive diagnosis in this group of patients was fiberoptic bronchoscopy/biopsy,followed by percutaneous lung puncture.Squamous cell carc inoma and small cell carcinoma were diagnosed mainly by fiberoptic bronchosco py/biopsy,and the proportion of confirmed adenocarcinoma cells by pleural effusi on cytology was significantly higher than other types.6.Of the 859 patients who could be clearly staged,776 patients were stage IIIB and above,accounting for 85.80%.The proportion of patients with stage IIIB an d above of adenocarcinoma and small cell carcinoma was higher.Conclusion 1.Compared with the past,in recent years,the distribution of pathological types in new cases of lung cancer in Department of respiratory medicine of our hospital has changed.Adenocarcinoma has risen to the first place than squamous cell carcinoma,followed by squamous cell carcinoma and small cell carcinoma.Male cases are still significantly more than women.Among male patients,squamous cell carcinoma is still more common,but there is no significant difference with adenocarcinoma,while female is dominated by adenocarcinoma.2.The peak age of onset rose to 60-79 years earlier.The pathological types of all ages were common with adenocarcinoma.3.Compared with adenocarcinoma,the occurrence of squamous cell carcinoma and small cell carcinoma is more related to smoking.4.Different pathological types of lung cancer have different clinical features,some of which have a certain clinical differential value.5.Fiberoptic bronchoscopy brushing/biopsy and percutaneous lung puncture are the main methods for the diagnosis of lung cancer.6.Adenocarcinomas and small cell carcinomas are more aggressive than squamous cell carcinomas and are more likely to metastasize.
