ObjectiveTo explore the role of serum visceral adipose tissue-derived serine protease inhibitor(vaspin),C reactive protein(C-reactive protein,CRP)and irregular chemokines(Fractalkine)in the diagnosis of coronary heart disease.Methods137 cases of coronary heart disease in the Seventh People's Hospital,Zhengzhou Seventh People's Hospital in December February 2016,were selected as the subjects of coronary heart disease,of which 53 were stable angina pectoris(stable angina pectoris,SAP)patients(SAP group),and 35 cases were unstable angina pectoris(unstable angina pectoris,UAP).(group UAP),49 patients with acute myocardial infarction(acute myocardial infarction,AMI)(group AMI),and 63 cases of healthy physical examination in our hospital at the same age at the same age group were selected as the control group.All patients were collected after admission to measure the height,blood pressure,body mass index(body mass index,BMI),total cholesterol(total cholesterol,TC),triglyceride(triglyceride,TG),high density lipoprotein cholesterol(high density lipoprotein),low density lipoprotein,and low density fat eggs Low density lipoprotein cholesterol(LDL-C)and C reactive protein(C-reactive protein,CRP)were used to determine phosphocreatine kinase(creatine kinase,CK),creatine kinase isoenzyme,lactate dehydrogenase,and lactate dehydrogenase,glutamic oxaloacetic transaminase(AST),and alanine transaminase(glutamic pyruvic transaminase,ALT);the cardiac function parameters were monitored with color Doppler ultrasound apparatus,including the end systolic volume of left ventricular(left ventrieular end-systolic volume,),and the end systolic inner diameter of the left ventricle.Ventricular end-stolic diameter,LVESD),left ventricular end diastolic diameter(left ventrieular end-diastolic diameter,LVEDD),and left ventricular end diastolic volume(left ventrieular end-diastolic).Content.All data were statistically processed with SPSS23.0 statistical analysis software,and all the above indexes were compared.Pearson correlation coefficient and Logistic regression analysis were adopted to analyze the correlation between serum vaspin,Fkn,CRP content and coronary heart disease,and vaspin,Fkn,CRP alone were calculated.And the specificity,sensitivity,negative predictive value and positive predictive value of combined detection of three components in coronary heart disease.Result(1)Compared with the control group,there were no significant differences in age,sex,incidence of hypertension,BMI,HDL-C and TG in the four groups(P>0.05),and TC and LDL-C were significantly higher than those of the control group.LDH,CK,CK-MB,ALT and AST were significantly higher than those of the control group.In group AMI and group AMI,LDH,CK,CK-MB,ALT and AST in group A were significantly higher than those in SAP and UAP group(P<0.05).(2)Compared with the control group,the heart function parameters of the coronary heart disease group were significantly increased(P<0.05),and the UAP group was significantly higher than that of the SAP and AMI group(P <0.05).(3)The serum levels of Fkn and CRP in patients with coronary heart disease were significantly higher than those in the control group,while serum vaspin levels were significantly lower than those in the control group.The content of serum Fkn and CRP in group AMI was significantly higher than that in group UAP and SAP group,but in group UAP,the serum vaspin content of AMI group was significantly lower than that of UAP and SAP group(P<0.05).(4)Person correlation analysis showed that the serum levels of Fkn and CRP in patients with coronary heart disease were positively correlated with the decrease of cardiac function and the index of myocardial injury,and the serum vaspin content was negatively correlated with the decline of cardiac function and the index of myocardial injury(P<0.05).(5)Logistic regression analysis showed that high levels of Fkn and CRP and low vaspin content were independent risk factors for coronary heart disease(P<0.05).(6)The sensitivity and negative predictive value of combined detection of serum Fkn,CRP and vaspin were significantly higher than those of three(P<0.05).Conclusion(1)in patients with coronary heart disease and serum Fkn,CRP content increased,vaspin content decreased significantly,the serum in patients with acute myocardial infarction Fkn,CRP increased and vaspin decreased more significantly,followed by patients with unstable angina pectoris,again for patients with stable angina pectoris,but after the two in the serum vaspin content had no significant difference;(2)the abnormal degree of serum Fkn,CRP and vaspin in patients with coronary heart disease is significantly correlated with the degree of myocardial injury and cardiac function.The high levels of serum Fkn and CRP and low vaspin content are independent risk factors for coronary heart disease.(3)combined detection of serum Fkn,CRP and vaspin has high sensitivity in diagnosing coronary heart disease.It can reflect the severity of coronary artery disease in patients with coronary heart disease,and has important clinical application value in the diagnosis and evaluation of coronary heart disease..
