ObjectiveThis study aims to explore the role of P53 and Ki67 in the prognosis of Her-2 negative Luminal B breast cancer by analyzing the expression of P53 and Ki67 in Her-2 negative Luminal B breast cancer,so as to better guiding clinical practice.MethodsThrough the case retrieval system,96 cases of inpatients at the First Affiliated Hospital of Zhengzhou University were randomly selected between 2010.9-2012.9.All patients did not perform any medical related treatment(including adjuvant chemotherapy,endocrine therapy,radiotherapy,etc.)before admission.After admission,the relevant preoperative examinations were completed and “modified radical mastectomy ” was performed.Complete cycle chemotherapy was given postoperatively;histopathological immunohistochemical results were in accordance with Luminal B,according to Her-2 expression status,divided into two groups: 1.Her-2 negative type: estrogen receptor(ER)and /or progesterone receptor(PR)positive,Her-2 negative,Ki67> 14%;2.Her-2 positive type: Estrogen receptor(ER)is positive,progesterone receptor(PR)is arbitrary,Her-2 is positive,and Ki67 is arbitrary.The Her-2 positive Luminal B type was used as a control group.The pathological immunohistochemical staining was used to detect the expression of P53 and Ki67 in 68 cases of Her-2 negative Luminal B breast cancer and 28 cases of Her-2 positive Luminal B breast cancer.Clinical data using SPSS22.0 software package on the relationship between P53,Ki67 and clinical pathological indicators and the two of the prognosis of breast cancer analysis,the results were p <0.05 showed statistical significance.Results1.Of the 96 patients with Luminal B breast cancer,68(70.8%)were in the Her-2 negative group and 28(29.2%)were in the Her-2 positive group.The clinical data of Luminal B breast cancer subgroups were not statistically different in age,menstrual status,histological grade,hormone receptor status,TNM stage,tumor diameter,histological classification,recurrence and metastatic sites.(p>0.05);However,there was a statistically significant difference in the axillary lymph node status between the two subgroups(p<0.01).2.In the Her-2 negative subgroup,P53 was positively expressed in 37 cases(54.4%),and Ki67 was highly expressed in 25 cases(36.8%).In the Her-2 positive subgroup,P53 was positively expressed in 20 cases(71.4%),Ki67 was high.The expression was 15 cases(53.6%).The positive rates of P53 and Ki67 expression were lower in the Her-2 negative subgroup than in the Her-2 positive subgroup,but there were no significant difference in the expression of P53 and Ki67 between the two subgroups(p>0.05).3.In the Her-2 negative Luminal B breast cancer,the expression of P53 was not statistically different from the age,menstrual status,histological grade,TNM stage,tumor diameter,axillary lymph node status(p>0.05),but with hormone receptors.The status was statistically significant(p<0.05);Ki67 expression was not statistically related to age and menstrual status(p>0.05),but was correlated with histological grade(p<0.001),TNM stage(p<0.001),tumor Diameter(p<0.01),hormone receptor status(p<0.01),axillary lymph node status(p<0.05)were all statistically significant.4.In the Her-2 negative Luminal B breast cancer,the correlation between P53 and Ki67 expression was not significant(p=0.095,r=0.041).5.In the Her-2 negative Luminal B breast cancer,comparison of Kapian-Meier curves and Log-rank test,P53 expression and disease-free survival time were not statistically significant(p>0.05);Ki67 expression and disease-free survival time were statistically significant(p<0.05).The disease-free survival time was significantly shorter in the Ki67 high-expression group and the low-expression group,indicating that Ki67 high expression is an indicator of poor prognosis.Conclusion1.The expression of P53 and Ki67 was lower in the Her-2 negative Luminal B breast cancer subgroup than in the Her-2 positive Luminal B breast cancer subgroup,P53 positive and Ki67 high expression may be related to disease invasiveness.2.In the Her-2 negative Luminal B breast cancer,the correlation between P53 and Ki67 expression was not significant;Ki67 high expression indicates poor prognosis,However,the prognostic value of P53 needs further validation.
