Clinical Study On Treatment Of Knee Osteoarthritis From The Liver And Kidney And Modified Decoction Of Duhuojisheng Intervention

Objective:(1)to explore the relationship between deficiency of liver and kidney and knee osteoarthritis(KOA),so as to provide more theoretical basis for the clinical guidance of traditional Chinese medicine.(2)observation of Duhuojisheng Decoction in the intervention of liver and kidney deficiency type KOA patients improved,quality of life,prognosis and safety situation,to further verify the close relationship between liver and kidney deficiency and KOA,provide more effective pharmacological treatment for KOA.Methods:(1)based on the basic theory of traditional Chinese medicine,the relationship between liver and kidney deficiency and KOA was discussed from the literature ancient books and the modern literature retrieval platform.(2)From January 2016 to December 2017,Guangxi University of TCM affiliated Ruikang Clinical Medical College of rheumatology outpatient and inpatient department of 80 cases of liver and kidney deficiency type KOA patients,using randomly divided into treatment group(Duhuojisheng Decoction)and control group(Glucosamine Sulfate Capsules),Course of treatment for 8 weeks,,respectively before treatment,after 8 weeks of treatment and follow-up(twelfth weeks)and recorded scores of liver and kidney deficiency type KOA patients with integral quantization and health scale(HAQ),by comparing the observed curative effect,symptom improvement,quality of life,prognosis and safety situation.Results:(1)The deficiency of viscera is the basis of KOA disease,especially liver and kidney,and the deficiency of liver and kidney plays an important role in the formation of KOA.(2)Clinical efficacy: After 8 weeks of treatment,the total effective rate of the treatment group and the control group was 89.1% and 77.7%,respectively,follow-up(twelfth weeks)treatment group,the total efficiency of the control group was 89.1% and 66.7%,the total efficiency of two groups had significant difference(P < 0.05),the treatment group were better in the control group.Internal comparison of various integral groups: the two groups before and after 8 weeks of treatment and follow-up(twelfth weeks)the clinical symptom score,total symptom score,HAQ score group compared the differences were statistically significant(P < 0.05),after 8 weeks of treatment and follow-up(twelfth weeks)were better than that of treatment before(P < 0.05),after 8 weeks of treatment and follow-up(twelfth weeks)compared to no significant difference(P > 0.05).The comparison between various integral groups: before treatment: two groups of clinical symptom score,total score and HAQ score compared with no significant difference(P > 0.05),after 8 weeks of treatment: the treatment group and the control group had no statistical significance in joint pain,joint tenderness,joint swelling,joint activities is limited to contrast difference(P > 0.05),treatment group and control group contrast between Flaccidity unabled and soreness and weakness of tibia and knees total score,HAQ score was statistically significant(P < 0.05),the treatment group than the control group;follow-up(twelfth weeks): the treatment group and the control group had no statistical significance in the joint pain and joint activities is limited compared difference(P > 0.05),the treatment group in the joint tenderness,joint swelling,Flaccidity unabled,soreness and weakness of tibia and knees,the total symptom score,HAQ score improved significantly better than the control group(P < 0.05).Conclusion:(1)The interaction between the liver and kidney determines the deficiency of the liver and kidney at the same time.The deficiency of the liver and kidney is more vulnerable to the deficiency of the blood and the deficiency of the bone,the deficiency of liver and kidney is closely related to KOA and plays an important role in the formation of KOA.(2)Duhuo-jisheng decoction can effectively improve the clinical symptoms and quality of life of liver and kidney deficiency type KOA patients,the clinical curative effect,which further confirmed the close correlation between deficiency of liver and kidney and KOA.
