The Clinical Observation Of Knee Osteoarthritis (Cold-dampness Type) Treated By Gubi Decoction

Objective:Applying "Gubi Decoction" in the treatment of cold-dampness type of knee osteoarthritis(KOA),observing patients' symptoms,signs,IL-1,OPG,OPG/RANKL changes before and after treatment,then to explore regulation mechanism of OPG/RANKL system on the patients by "Gubi Decoction",and provide a new way for the treatment of KOA patients.Methods:60KOA patients,collected from Shanxi province hospital rheumatism outpatient and inpatient department,were randomly divided into treatment group and control group.The treatment group was treated by traditional Chinese medicine "Gubi Decoction" orally,the control group by oral administration of Glucosamine Hydrochloride Capsules and Diacerein Capsules,for 12 weeks.Before and after treament,recording TCM symptoms integral,WOMAC score,blood test,urine test,liver and kidney function,IL-1,OPG,OPG/RANKL ratio,then statistical analysis of date.Results:On the TCM symptoms integral,there was no difference between the two groups before treatment but after treatment,the scores of TCM symptoms in the two groups all reduced after treatment,the treatment group was significantly lower than the control group,there was significant difference(P<0.05).WOMAC score of two groups before treatment had no significant difference(P>0.05)but after treatment(P<0.01),the treatment group was better than the control group.There was no significant difference in IL-1 before treatment(P>0.05)but after treatment(P<0.01),the treatment group was better than the control group.There were no significant difference on OPG and OPG/RANKL before treatment(P>0.05)but after treatment,there was significant difference comparing with no-treatment on OPG(OPG/RANKL)(P<0.01).The total effective rate of the treatment group was 93.33%,while which of the control group was 83.33%,there was significant difference of total effectiveness(P<0.05).There was no statistical difference in the safety of the two groups.Conclusion: "Gubi Decoction" had more obvious effect in the treatment of KOA in inproving clinical symptoms,reducing the inflammatory factor IL-1 than glucosamine hydrochloride combined with diacerein.They all can increase OPG and OPG/RANKL in serum,inhibit bone from resorption,thereby protect the knee cartilage.
