The Application Reaserch Of Two Types Of Vsccular Graft On Femoral-popliteal Arteriosclerosis Obliterans

Objective:To analyze comparatively the medium-long-term curative effect of ePTFE artificial blood vessel and autogenous saPhenous vein as the transplants for arterial byPass that to treat femoral and PoPliteal arteriosclerosis obliterans.Methods:All the 40 cases of the second affiliated hosPital of Hunan Normal university between December 2010 and December 2015(40 limbs)with femoral or PoPliteal arteriosclerosis obliterans Patients’clinical data had been retrospectively analyzed.According to the different intraoPerative grafts,40 cases were diveded into the artificial vascular byPass flow grouP(grouP ePTFE)and uPside down and great saPhenous vein byPass flow grouP(grouP SV).The ABI values,Color DoPPler ultrasound Patency rate and other datas of Postoperative follow-uP in 1～24 months were collected and analyzed statistically.Results:The operations for all 40 cases in A.B grouPs were successfully,no serious complications or PerioPerative deaths.Only 1 case of wound effusion in ePTFE grouP at the 6th day after the surgery,no complications emerge in SV grouP.All the case had been done the Postoperative follow-uP at 1st month,3rd month,6th month,12th month and 24th month after the surgery.22 cases in ePTFE grouP,the average ABI value,Patency rate before the surgery was 0.31±0.11 and 13%,at the 1st week,1st month,3rd month,6th month,12th month and 24th month after the surgery,the average ABI value turns to 0.98±0.08.0.97±0.05、0.95±0.11、0.92±0.06、0.90±0.09、0.89±0.09 resPectively,and the Patency rate turns to 100%、100%、100%、95.5%、90.5%、83.3%.18 cases in SV grouP,the average ABI value,Patency rate before the surgery was 0.35±0.09 and 15%,at the 1st week,1st month,3rd month,6th month,12th month and 24th month after the surgery,the average ABI value turns to 1.08±0.06、0.99±0.08、0.97±0.09、0.95±0.07、0.94±0.12、0.90±0.08 resPectively,and the Patency rate turns to 100%、100%、100%、100%、94.4%、88.2%.The difference of 2 grouPs’ average ABI values and Patency rate has no statistically significant,P>0.05.3 limbs of 3 cases in two grouPs had been amPutated during the Period of Postoperative follow-uP,one limb of blow-knee amPutation and another of above-knee amPutation in ePTFE grouP,one limb of above-knee amPutation in SV grouP,the limb salvage rate during 2 years was 92.5%(37/40).Conclusion:Two tyPes of transplants for arterial byPass that to treat femoral and PoPliteal arteriosclerosis obliterans are both eutheraPeutic,and the Patency rates in 2 years reach above 80%,are higher than domestic corresponding Period.The PostoPeration ABI value and the Patency rate of using autogenous saPhenous vein as the transplant are curatively better ComPare to the data of ePTFE artificial blood vessel.In conclusion,autogenous saPhenous vein is the Preferred alternative when it available.
