Object:To explore the changes of vascular endothelial function in the different body parts of patients with arteriosclerosis obliterans(ASO), furthermore, to research the correlations of the vascular endothelial function and Ankle-brachial index(ABI), to explore the correlations between the vascular endothelial function and TCM syndrome types of ASO.Methods:Selecting 115 cases of patients with ASO as observation group, and taking 60 cases of the healthy voluntary persons as the control group. Using high frequency ultrasonic testing Flow-mediated dilation (FMD) of brachial artery(BA), superficial femoral artery(SFA) and deep femoral artery(DFA), and measuring ABI, common carotid artery(CCA) and common femoral artery(CFA) intima-media thickness(IMT) of each group. Focus on the correlation between vascular endothelial function in different testing parts and ABI. With ABI boundary values, to determine the cut-off value of the vascular endothelial function in different testing parts. On this basis, the ASO patients were divided into two groups based on TCM called blood stasis type and damp-heat type, to discuss the correlations of them and TCM syndrome types.Result:1. The values of ABI and FMD in ASO patients were lower than control (P <0.01),but the values of CCA-IMT and CFA-IMT were higher than control (P<0.01)? 2. There are positive correlations between the values of FMD on BA, SFA, DFA and ABI (P<0.01), the range of Pearson correlation index is 0.459-0.513 in patients, as well as 0.394?0.542 in control.3.ABI as threshold value, the cut-off values of FMD on BA? SFA?DFA were 6.23%?5.25% and 5.57% respectively.4. The values of ABI and FMD in damp-heat syndrome group were lower than control (P<0.01), but the values of CCA-IMT and CFA-IMT were higher than control (P<0.01).5. Three indicators which had a greater influence on the TCM Syndrome through regression analysis were CFA-IMT, DFA-FMD and ABI. The values of the OR were 13.525,0.569 and 0.003, respectively.Conclusion:1. Assessment of vascular endothelial function in patients with ASO by ultrasound was effective.2. ABI as threshold value to study the degree of impaired endothelial function and to determine the cut-off value was feasible.3. The indicators of Ultrasonic evaluation can provide objective evidence for microcosmic syndrome differentiation of traditional Chinese medicine.
