Background:With the rapid development of the computer and communication technologies,database system is rapidly becoming an essential part of the modern world.Database which has a nice action of management and sharing are widely used in industries and has become an important tool of data management.Now traditional medical gradually changed into evidence-based medicine,which need more clinical practice and more research data.It involves data of so many categories and in a so huge number that manual processing is both time and energy consuming,liable to run into errors as well.In order to realize storage,query,analysis and sharing of clinical data,clinical database have sprung up.Clinical database provide vital resources and technological tools for high quality medical education and research.A complete clinical database involves a considerable number of physician practices,which cover epidemiological information,demographic profiles,initiation and development of diseases,laboratory test results,diagnosis and treatment,prognosis and follow up and help to analysis development pattern of diseases,screen patients to study and improve quality of medical care.Paraquat poisoning had the largest percentage of patients which were treated in our department.Paraquat is widely used in farming with its convenience and better effect than other herbicide.Nowadays,there are more and more cases of paraquat poisoning,which are often caused by suicide or accidental ingestion.Paraquat has high toxicity and there are no known pharmacological antagonists for its poisoning at present.The damages of paraquat are in multisystem and multiple organ such as liver,lung,kidney,heart and brain.Respiratory failure caused by pulmonary fibrosis is the major cause of death.However,the exact mechanism of its toxicity still remains unclear.There are many gaps and deficiencies in the researches of relations between paraquat poisoning and multisystem organ,which needs to be more explored.Objective:To establish a clinical database of acute poisoning,which can meet the requirement of clinic and research.Using this database to storage,query,analysis and sharing clinical data.To analyse retrospectively the clinical features,laboratory tests and treatment of patients with acute poisoning taking treatment in our hospital in 2016.To discuss population characteristics and prognostic indicators of paraquat poisoning,thus to provide basic data for improving treatment measures and follow-up record.Method:Established questionnaire of acute poisoning,which included informed consent,general information,clinical information,prognosis and follow-up of patients.Built up the acute poisoning database with epidata,collected demography and clinical data of patients and input to the database.The data was statistically analyzed with SPSS.The method of variance analysis was employed to analyze qualitative data,and quantitative data was analyzed by mainly using the method of t test and rank test.If p<0.05,the difference of the sensitivities was significant.Results:(1)625 cases of acute poisoning were treated in our department in 2016.Most patients are young adults.The results showed no significant difference in gender grouped by age.The herbicides poisoning ranked first,paraquat poisoning and glyphosate poisoning had the largest percentage of patients.(2)403 cases of paraquat poisoning were treated in our department.Most patients are between the age of 30 and 39.The results showed no significant difference in gender grouped by age.Most of patients were farmers.As the vulnerable group of the social,their education is generally in the low level,and most of them are junior middle school education level.Some patients had a history of mental problems or suicide.Most patients came from Jinan,Dezhou,Qingdao in Shandong Province.The number of patients has been rising since summer,then gradually decreasing,had a peak at 5?8 months.(3)Patients were divided into two groups which are death group and alive group.The results showed that gender of the two groups was no significant difference(p>0.05),while age and the amount of poison were statistically significant(p<0.05).Compared two groups of patients with early biochemical indexes,the results showed that Hb?PLT of the two groups were no significant difference(p>0.05),while WBC?N?ALT?AST?AKP?LDH?TBIL?DBIL?IBIL?Cr?BUN?CyS-C?UA?CK?CK-MB?cTnI?AMY?LIP?DD-iwere statistically significant(p>0.05).The risk factors of poor outcome were WBC?Cr and LDH.Conclusion:The study has successfully established the acute poisoning clinical database,the clinical application confirms that the database has good practicality.The database has certain reference value and realistic meaning to the development of acute poisoning.
