| Objective: To evaluate the influence of different purification methods on prognosis in patients with acute paraquat poisoning.Methods: Use the method of retrospective study,a total of 89 patients with acute paraquat poisoning were divided into three groups according to different treatment methods:conventional treatment group,HP group,and HP + CVVH group.The date of patients were collected and analyzed from hospital’s electronic database after hospital admission,such as the result of laboratory tests,survival time of the deaths and prognosis.Multiple logistic regression model was used to evaluate the characteristics of the dead patients with acute paraquat poisoning.Results: There was significant difference in mortality among three groups(?2=10.73,P=0.005),the mortality of HP group was obviously lower than the control group(?2=6.49,P=0.011)and the mortality of HP+CVVH group is significantly lower than the control group(?2=10.53,P=0.001).But there was no significant difference in mortality between HP group and HP+CVVH group(?2=0.59,P>0.05);There was significant difference in survival time between three groups(F = 39.79,P < 0.01),and the survival time of HP + CVVH group was higher than the HP group and the control group(P<0.05).The multiple logistic regression analysis indicated that paraquat ingestion dose [odds ratio(OR)= 1.169,95% confidence interval(95%CI)= 1.039~1.315,P < 0.01];time from admission to HP [odds ratio(OR)= 3.635,95% confidence interval(95%CI)= 1.535~8.608,P < 0.01];WBC [odds ratio(OR)= 1.530,95% confidence interval(95%CI)= 1.115~2.100,P < 0.01] were the risk factors correlated with the prognosis of patients with acute paraquat poisoning.Conclusion:The blood purification method of HP combined with CVVH can effectively reduce the multiple organ damage caused by poisoning and prolong the survival time of patients with poisoning.However,no significant difference has been found in reducing the long-term mortality rate of the method compared with that of HP.Paraquat ingestion dose,time from admission to HP and leukocyte count were all related to prognosis of the patients. |
