| Objective:1.The purpose of this study is to investigate the status of post-traumatic growth,psychological resilience and quality of life in young patients with hemodialysis and the factors that affect it.2.To explore the correlation between the resilience of young hemodialysis patients and their post-traumatic growth and quality of life,to provide new ideas for the treatment and psychological intervention of young hemodialysis patients,and to take effective nursing measures to improve the quality of life of young hemodialysis patients.Method:Using the method of convenient sampling,200 cases of maintenance hemodialysis patients were selected from the blood purification room of six third class hospitals in Shandong province from May 2017 to October 2017.The self-designed general questionnaire,social support scale,Mental Resilience Scale(CD-RISC),post traumatic growth scale(PTGI)and quality of life scale(SF-36)were used to investigate the related data,social support status,psychosoelasticity,posttraumatic growth status,and quality of life.Use Excel Chinese version to record data and check the validity of forms.SPSS19.0 test data,measurement data in(mean + standard deviation)form description(x + s).For the comparison of general demographic data,two independent samples were used for test.The significant level was expressed as P<0.05,and the difference was statistically significant.The relationship between the three factors was analyzed by multivariate Logistic regression analysis.Results:The survey sent a total of 210 questionnaires,200 were recovered,the questionnaire was 95.2%effective recovery rate.The concrete results are as follows:1.The total score of psychological resilience(62.32 + 8.67)is lower than that of ordinary people surveyed by medical researchers(65.4 + 13.9).The scores of each dimension were:optimism(13.67±3.21),toughness(8.93± 1.70),and strength(39.72 ± 3.76).Among them,the optimism dimension is higher than the other two dimensions,which shows that patients' expectations for disease cure are more optimistic.The lower score of the strength dimension indicates that the patients adapt slowly to the hospitalization events of maintenance hemodialysis.The minimum score of tenacity indicates that the patient is not strong in the physical discomfort of the disease.It may be caused by the treatment of the disease.The patient is hospitalized for a long time or in bed in the home,and the activity is reduced,which leads to the inconveniences of the patient's daily life and the end of the patient's low endurance.2.Three dimensions of social support score were:friend support(13.76 ±2.65),family support(20.63±2.75),and other support(13.78±1.52).Among the three dimensions,the highest score of the family support dimension showed that the family gave the most support to the patients,and the friends and other aspects(such as the medical staff and so on)gave less emotional help.3.The total score of posttraumatic growth(68.26 + 17.54),higher than(64.23 +18.40)of the domestic researchers for breast cancer analysis results,in the middle and higher level,the scores of the various dimensions did not show significant differences.4.Measurements of the quality of life of the young maintenance hemodialysis patients showed that the scores of the patients in the maintenance hemodialysis patients were lower than the domestic norm in the physiological functions,physiological functions,overall health,life vitality,social function,emotional function and mental health,while the score of body pain dimension was higher than the domestic norm.There were significant differences,the differences were statistically significant(P<0.05).5.The findings showed that there was a significant correlation between resilience,post-traumatic growth and quality of life in young maintenance hemodialysis patients.There is a positive correlation between mental resilience and post traumatic growth,and there is a positive correlation between personal strength,self change,and the relationship with others,and there is no correlation between the new possibility,the life perception dimension and the quality of life in the posttraumatic growth,and the results and birth of the three dimensions of mental elasticity.The physiological function,physiological function,general health,vitality,social function,emotional function and mental health of living quality have a significant positive correlation.6.Stratified multiple regression analysis showed that the results of the quality of life of the patients with maintenance hemodialysis showed that family income,medical payment,course of illness,mental resilience,personal strength,self change,and the relationship with others entered the regression equation,and 57.3%of the changes were explained.The higher the family income of the young maintenance hemodialysis patients,the lower the duration of the disease,the higher the resilience of the patients.In the five dimensions of posttraumatic growth,personal strength,self change and relationship with others have entered the regression equation.This indicates that posttraumatic growth plays an important role in the quality of life of young maintenance hemodialysis patients.Conclusion:1.Through this survey,it was found that the mental resilience and quality of life of the patients in the maintenance hemodialysis patients were at the middle level,and the posttraumatic growth was in the middle level.2.There were different levels of relationship between the levels of mental resilience,PTG and quality of life among the three maintenance hemodialysis patients,and the mental resilience of the maintenance hemodialysis patients and the predictive effect of PTG on the quality of life in the patients with maintenance hemodialysis.3.The resilience level and posttraumatic growth of young maintenance hemodialysis patients can significantly predict their quality of life.The improvement of resilience and measures to increase post-traumatic growth in patients can improve the quality of life. |
