Objective To investigate the status of post-traumatic growth,post-traumatic stress disorder and the quality of life in elderly patients with hip fracture and then analyze the influence of post-traumatic growth,post-traumatic stress disorder on the quality of life,and to further explore influencing factors of quality of life in elderly hip fracture patients,so as to provide scientific basis for improving the quality of life in elderly patients with hip fracture.Methods Face-to-face survey was conducted in 486 patients with hip fracture 3 months after surgery in the Third Hospital of Hebei Medical University between October 2015 and September 2016 using a SF-36 quality-of-life questionnaire,a posttraumatic growth scale,a symptom self-rating scale of posttraumatic stress disorder,a self-rating depression scale,a social support rating scale,a questionnaire for Harris hip score and a timed standing-walking test in outpatient reexamination.All questionnaires were withdrawn immediately after filling in and examined in time.All data were subjected to the construction of database using Excel.SPSS 22.0 was applied for statistical analysis.Applied statistical methods included descriptive analysis,t test,analysis of variance,Pearson correlation analysis and multiple linear regression analysis.Results 1 Among the elderly patients with hip fracture,quality of life was in a medium/low level,with score ranging from 29 to 85(average score,52.78±10.54);the average score of posttraumatic growth was(63.68±10.02),with patients in medium/high growth level accounting for 62.6%;the positive rate of posttraumatic stress disorder was 10.3%.2(1)Gender,age,degree of education,preoperative anemia,the timing of surgery,surgical procedures and the incidence of complications influenced patients' quality of life,presenting statistical significance(P<0.05);(2)Poorer function of hip joint and longer duration of walking ability test were correlated with poorer quality of life in patients(P<0.05);(3)Higher degree of depression and lower level of social support were correlated with poorer quality of life in patients(P<0.05);3 Correlation analysis demonstrated that posttraumatic growth was positively correlated with patients' quality of life(r=0.571,P<0.05),and posttraumatic stress disorder showed a negative correlation with patients' quality of life(r=0.355,P<0.05);4 Multiple linear regression analysis revealed that female,high age,low degree of education,delayed timing of surgery,internally fixed surgery,high degree of depression,low level of social support,poor function of hip joint,long duration of walking ability test,low level of posttraumatic growth,severe posttraumatic stress disorder led to patients' poor quality of life.Conclusion 1 Among the elderly patients with hip fracture,quality of life is in a medium/low level 3 months after surgery.2 In the elderly patients with hip fracture,posttraumatic growth is positively correlated with their quality of life,and posttraumatic stress disorder is negatively correlated with their quality of life.3 Multiple linear regression analysis shows that gender,age,degree of education,the timing of surgery,surgical procedures,depression,social support,the function of hip joint,walking ability,posttraumatic growth and posttraumatic stress disorder are influencing factors of quality of life in elderly patients with hip fracture.
