ObjectivesTo investigate the distribution status of relevant radiation practitioners in 31 provinces(municipalities,autonomous regions)in China,and to understand the current situation of the allocation in various provinces(municipalities,autonomous regions).The Gini coefficient was used to analyze and evaluate the fairness and the scientific status of the status quo of radiation-related personnel from the perspective of population and geography.The Theil Index was used to measure inter-regional and intra-regional fairness,and the influencing factors were also analyzed.To understand the fairness of the distribution of relevant radiation practitioners in various regions and provinces in China,and provide basis for the government health and family planning department to formulate scientific and reasonable health personnel allocation standards.MethodsThe questionnaires were compiled by means of literature analysis and expert consultation in this study,and statistics were collected from relevant health and family planning institutions in each province.The survey included hospital-related information(hospital level,hospital type),basic information of relevant radiation practitioners(gender,age,job title,position,length of service,etc).The collected data was checked one by one and the missing information was called for registration.Created a database using Excel 2013.Statistical analysis was performed on the relevant indicators using the statistical software SPSS 21.0.The Gini coefficient and the Theil index were calculated in Excel 2013.Results1.The situation of the distribution of radiation workers in China.In the eastern region,the number of radiologists per 10000 population was 0.66,which was much higher than that of 0.48 and 0.36 in the central and western regions.The number of radiation practitioners per 10000 population in the eastern and central regions was more than 0.5,and the eastern region was in an average state as a whole.Radiological medical personnel resources were adequate.Although the total number of radiologists in the central region was slightly lower than that in the eastern region,the population base was far higher than the eastern region by 27%,and the number of radiologists per 10,000 people was basically between 0.2-0.4.2.The distribution of relevant radiation workers in each province(municipalities,autonomous regions)in China based on the Gini coefficient of population was below 0.3,which was on the best condition.According to the geographic Gini coefficient,radiologists,technicians,and care settings were all in the range of 0.4-0.6,which was in a state of alert.The geographical distribution of the Gini coefficient was relatively high.The fairness of population distribution was better than that of geographical distribution.3.The Theil index of radiologists,technicians,and nurses was 0.1936,0.2095,and 0.2270,respectively in the distribution of relevant radiation practitioners in various provinces(municipalities,autonomous regions)in China,and the overall allocation was relatively fair.The inter-regional Theilindex of radiologists,technicians,and nursing staff was higher than that intra-regional Theil index,which indicates that the allocation of human resources of relevant radiation practitioners in the eastern,central,and western regions was unfair.The contribution rate of the inter-regional distribution of human resources of related radiation workers was greater than the contribution rate within regions,which means that the inter-regional differences were the main factors that influence the distribution of the total Theil index of the relevant radiation employees in eastern,central and western part in China.4.The concentration index of radiologists,technicians,and nurses in hospitals above Grade 2 in each province(autonomous region and municipality directly under the Central Government)were 0.0494,0.0287,and 0.0077,respectively,which indicates that the overall distribution of related radiation practitioners in hospitals in Grade 2 or above in China was relatively good.Radiation care staffing was more equitable than radiologists and technicians.However,the configuration of radiologists and technicians was more inclined to the regions with higher economic levels,and the distribution of relevant radiation practitioners between regions with high economic levels and those with low economic levels was less fair.Conclusions1.Radiation practitioners are an important part of medical and health resources.The fairness of human resources allocated to population by radiation workers in China is better than that based on geographical distribution.The number of radiologists is concentrated in better economic regions.2.The equitable distribution of radiation workers in the eastern,central and western regions is affected by the demographic and economic status of the region.Although the internal fairness of the region is good,the difference between regions is the main factor affecting the fairness of human resources allocation of radiation practitioners,and it also indirectly reflects the problems existing in the distribution of health resources in the region.3.The distribution of health resources in our country presents an "inverted triangle" phenomenon.When formulating a regional plan for health,the government should not only increase the investment in hardware facilities,but also pay attention to the introduction of human resources for health and achieve sound development.In addition,consideration should be given to economic development and the geographical distribution of medical resources.Targeted policies should be formulated for the allocation of human resources related to radiology,and corresponding measures such as “hospital support”,“co-cultivation”,“regular communication” and “circulatory consultation” should be adopted.We will increase the subsidy policy for relevant talents in areas with weaker human resources for health and do a good job of talent introduction programs.It adopts incremental control measures to guide it to optimize the allocation between regions and regions and achieve a fair allocation.
