The Research Of Chronic Care Service Systems For Elderly People

The serious problem of Aging has drawn widespread attention from all sectors of society day by day,the health of the elderly has been attached more and more people’s attention.The biggest obstacle To the health of the elderly life is chronic disease,It has been a valuable subject that combine chronic care and elderly people ’ s daily life together.The purpose of this study is to set up a service system fits the actual needs of the old people and their lifestyle to adapt to chronic care service system for the elderly in community,to provide convenient services for chronic care of the elderly,to fit the the elderly’s psychological expectations and service demand in long-term care as much as possible.Using the method of literature research,depth interviews,non participant observation,user research,correlation analysis and service design practice through literature retrieval method,summarizing the condition of community home care for chronic diseases by searching related literature papers,and combing the concept of basic service design,as a theoretic preparation for further research on methods and processes.By planning the research,designing investigation tool for the elderly with chronic diseases of non participant observation,following and recording specific lifestyle of the elderly in chronic disease care,and interviewing community departments that involved,understand the overall system status of elderly chronic care.After combing the internal needs,motivations and expectations of the chronic disease elderly’s critical behavior,summed up the core psychological needs of the elderly chronic disease care.Through the methods of user research and service design,the relationship and interaction between stakeholders in the lifestyle of chronic diseases in the elderly were sorted out.Then,combined with the specific information of the two target users,we constructed the persona of the elderly with chronic diseases.Based on personas,analyzed scenarios of elderly that in long-term chronic disease care,including process of chronic disease drug dosage adjustment,context of information interaction,and scenario of chronic disease rehabilitation exercise of the elderly.Accordingly,five design opportunities are proposed.The first one is the system integration of the chronic patient’s personal health information;the second is to reduce the number of elderly’s outpatient,to ensure long-term continuous care by drug delivery service design;the third one is drug management and medication remind mechanism in the unit of family;the fourth is therehabilitation exercise service based on community chronic disease nursing platform;the fifth one is a game based chronic disease engagement system.In the practical design phase according to the research,established an integrated service concept framework of information system for people in chronic disease,constructed drug delivery service system module,shown by the specific process by service blueprint and storyboard.This paper explores the product design of drug management terminal in the family and self-help drug delivery box in the community.
