Radiation dermatitis refers to the inflammation of the skin and mucous membrane of the irradiated field caused by the large radiation energy(mainly ?,? and X ray)during the radiotherapy process,which can be shown as hair loss,skin pigmentation,flushing,dry and wet desquamate,skin tenderness and even ulcers.Radiation dermatitis seriously affects patients' physical and mental health,and reduces patients' quality of life and compliance of continuous treatment.Western medicine has made some progress in the treatment of radioactive dermatitis,but it can not get rid of the expensive drawbacks.The purpose of this paper is to expound the research progress of radiation dermatitis and to provide the theory for the treatment of radiation dermatitis by traditional Chinese medicine.This thesis includes:literature review?literature research and clinical research.Part 1.Literature Review and Literature ResearchThe article reviews the research progress of Chinese and Western medicine in the field of radiation dermatitis in recent years and mainly focuses on the aspects of epidemiology,grading standards,clinical manifestation,mechanism research,for the purpose of understanding Chinese and Western medicine on radiation dermatitis,and the progress of studies on the treatment of radiation dermatitis by Chinese and Western medicine.In addition,the article also systematically incorporated 37 articles on clinical trials of radioactive dermatitis for external use in the past 15 years,50 kinds of traditional Chinese medicine were summarized to have been frequently used in treating radioactive dermatitis and an analysis made on each of its clinical drug characteristics.Part 2.Clinical StudiesObjective:The research aims to observe the clinical effect of external application of traditional Chinese medicine ulcer oil(main ingredients:Lithospermum erythrorhizon,Safflower,Astragalus,Angelica and Rhubarb)in the treatment of acute radiation dermatitis.Specifically,this study aims to study its effectiveness on reducing the degree of dermatitis(RTOG)and the skin pain score of the radiated field(NRS),and to verify its effects on heat-clearing and detoxifying,heat removing,bleeding stopping,swelling and pain relieving,wound convergence and muscle regenerating-in treating burns and ulcers so as to provide scientific research for the treatment of radiation dermatitis with traditional Chinese medicine,and to provide a high quality,inexpensive external medicine for clinical treatment.Method:This study adopted non-randomized controlled method,and initiated multi-center research team consisted of 8 institutions 52 patients with radiation dermatitis that met criteria were selected as subjects under this study.The patients were divided into experimental group(25 cases)and control group(27 cases).The experimental group was given conventional nursing and traditional Chinese medicine ulcer oil for external use,and the control group was given conventional nursing combined with a Western medicine(Biafine;Kangfu,Kangfuxinye)for external use.Both groups of patients used drugs twice a day and observed for 14 consecutive days.Observed indicators are graded by radiation dermatitis(RTOG classification),skin field pain score(NRS),effectiveness before and after treatment.The safety evaluation indicators include blood routine,liver and kidney function,and electrocardiogram.The statistical method was analyzed using SPSS 22.0.Results:1.After 14 days of treatment,the comparison of the efficacy of radiation dermatitis(RTOG)in the experimental group and the control group was similar(P>0.05),which was not statistically significant,but the effective rate of the drug in the experimental group was slightly higher than that of the control group;2.The degree of radiation dermatitis grade(RTOG)in the experimental group after 14 days was compared with that before treatment(P<0.05).The rank mean of RTOG grading after treatment was less than that before treatment.3.The level of radiation dermatitis(RTOG)in the control group was significantly different from that before treatment(P<0.05)after 14 days of treatment(P<0.05),and the rank mean of RTOG grading after treatment was less than that before treatment.4.After 14 days of treatment,there was no significant difference in skin pain score(NRS)between the experimental group and the control group(P>0.05).5.The control group was statistically significant(NRS<0.05)after 14 days of treatment and before treatment(P<0.05),and the mean value of NRS score after treatment was less than that before treatment.Conclusions:1.Chinese traditional medicine ulcer oil is effective in reducing the degree of acute radiation dermatitis(RTOG);2.Chinese medicine ulcer oil treatment of radiation dermatitis and western medicine are roughly the same;3.Traditional Chinese medicine ulcer oil in the reduction of radiation field skin pain score(NRS)is invalid.
