| Purpose:To observe the efficacy of Qingjinhuatan Decoction combined with hormone in the treatment of acute radiation pneumonia.The pathogenesis of traditional Chinese medicine of acute radiation pneumonia for the heat evil in lung and deficiency of qi and yin,according to the symptoms of patients with radiation pneumonia for diagnosis and treatment.This trial therapy for radiation pneumonia by Qingjinhuatan Decoction combined with hormone to explore and analyze the variation of clinical symptoms,KPS score,blood TGF-?1 level,lung function,chest CT.Material and method:The design of clinical trial with single-blind prospectie randomized control,60 patients with acute radiation pneumonia in Ansteel General Hospital from October 2014 to October 2016.The patients of radiation pneumonia who were deficiency of qi and yin.The patients were divided into experimental group and control group,patients were eligible for inclusion criteria and did not meet the exclusion criteria,the patients participated in the trial voluntarily and signed the informed consent form.Patients in the control group prednisone 30-50 mg/d orally,the experimental group was treated with prednisone 30-50 mg/d and Qingjinhuatan Decoction.Both groups were treated for 4 weeks.To evaluate the efficacy by the lung function,blood TGF-?level(ELISA)and of chest CT and the quality of life score(KPS).Finally,statistical analysis was performed by SPSS 21.0.Results:Total effective rate: After treatment,the experimental group of patients met the effective standard 28 cases,ineffective 2 cases,the total effective rate of the experimental group was 93.33%.The control group of patients met the effective standard 22 cases,ineffective 8 cases,the total efficient of the control group was 73.33%.The total effective rate of the experimental group was higher than the control group,The difference was statistically significant by ?2 test(P <0.05);The level of TGF-?:The level of TGF-?of the two groups before treatment were(37.90±4.91)and(38.71±4.01).The level of TGF-?of the two groups after treatment were(26.24±5.37)and(30.26±5.51).The level of TGF-?of the experimental group was lower than control group significantly,the difference of blood TGF-?1 between the two groups was statistically significant(P<0.05).KPS score:The KPS score of the two groups before treatment were(67.00±5.96)and(65.00±5.72).At the end of treatment,the KPS score of the experimental group and the control group were(85.00±8.20)and(78.30±10.31).KPS score of the experimental group and control group after treatment were higher than before treatment,the difference was statistically significant(P<0.05),KPS score of the experimental group was higher than the control group after treatment(P<0.05);Lung function: Before treatment,the FEV1 was(1.33±0.13)and(1.31±0.15)in the experimental and control groups,FEV1% was(64.03±2.10)and(63.11±1.68).The FEV1 was(1.86±0.28)and(1.57±0.25)in the experimental and control groups after treatment,FEV1% was(76.35±4.50)and(71.50±4.31).After treatment,the recovery of lung function in the experimental group was significantly better than the control group,there was a significant difference between the two groups in the recovery of lung injury caused by radiation pneumonia(P < 0.05);Conclusion: 1.Qingjinhuatan decoction combined with prednisone treatment of radiation pneumonia was effective,improved the patient's quality of life significantly,anesised the patient's clinical symptoms significantly,improved the patient's lung function significantly,the effective was better than prednisone treatment of radiation pneumonia.2.The treatment of Qingjinhuatan decoction combined with prednisone can inhibited the TGF-?1 level significantly,further inhibit aggregation and infiltration of immune factors in the patients,to reduce the patients with lung tissue inflammation,make the patient's lung injury minimum. |
