Research On Ancient And Modern Medication Thinking Of Diabetic Nephropathy

Objective:Diabetic nephropathy is a common complication of diabetes,belonging to the category of traditional medicine "kidney consumption",Chinese medicine play an irreplaceable role in its treatment.In this paper,diabetic nephropathy was regarded as research object,the ancient and modern prescriptions and medical records of diabetic nephropathy were collected systematicly,the features medication and compatibility law of past and present treatment of diabetic nephropathy were statisticed and analyzed,the treatment principles and cogitation of Chinese medicinals of ancient and modern treatment of diabetic nephropathy were compared in order to provide a reference for clinical application.Methods:Chinese Medical Classics and family medicine network database were regarded as retrieval approach to retrieve the ancient prescriptions and medical records of diabetic nephropathy;China National Knowledge Infrastructure(CNKI),Wanfang database,China Science and Technology Journal Database,China Academic Conference Paper and China degree thesis were regarded as search databases to search modern treatment of prescription for diabetic nephropathy.Basic statistical methods were applied to analyze the frequency,dosage and proportion of medicinals in the prescription of diabetic nephropathy in various Dynasties.The higher frequency medicinals in each period was calculated.The treatment principle and cogitation of Chinese medicinals of diabetic nephropathy in each period were analyzed.Results:Overall,the Dynasties before Ming in which warm medicinals were used more,the proportion of warm medicinals and cold medicinals was almost the same,neutral medicinals were used less for the treatment of diabetic nephropathy;The Ming Dynasty to the Qing Dynasty and the Republic of China cold medicinals were used more than the warm medicinals,neutral medicinals were used less for the treatment of diabetic nephropathy;In the modern times the proportion of warm medicinals and cold medicinals had no obvious difference,neutral medicinals were used less.In terms of the taste,the proportion of sweet medicinals was the most in ancient times for the treatment of diabetic nephropathy,which was far more than others in each dynasty,The proportion of sweet-cold medicinals,sweet-neutral medicinals and sweet-warm medicinals were more,and had no obvious difference,which followed by bitter medicinals,that the proportion of bitter-cold medicinals was larger,and increased in each dynasty;pungent medicinals and acid medicinals were used less,the Qing Dynasty and the Republic of acid medicinals were used more than pungent medicinals.The proportion of sweet medicinals was largest in modern times for the treatment of diabetic nephropathy,followed by bitter medicinals,which the frequency and dosage of sweet-neutral medicinals,sweet-warm medicinals,bitter-cold medicinals were larger,while,sweet-cold medicinals was used less than the ancient times,pungent medicinals and acid medicinals were used less than sweet and bitter medicine.In terms of the efficiency,tonic medicinals was used the most both in ancient and modern times,followed by the heat-clearing medicinals,astringent medicinals and desiccant medicinals also accounted for a certain proportion;what's more,the frequency of tonic drugs in the Qing Dynasty and the Republic of China was largest;The proportion of heat-clearing medicinals increased after the Ming Dynasty and declined in modern times;The proportion of astringent medicinals was the largest in the Song-Jin-Yuan Dynasties,which was uniform in other times;The proportion of anti-dampness medicinals in the Qing Dynasty and the Republic of China was the least,which was used more in modern times;And the use of blood-activiating medicinals in modern times was far more than ancient times.Overall,the treatment of diabetic nephropathy in ancient times focus on nourishing kidney vitality,essence and blood,clearing heat and nourishing Yin,and to clearing damp,reinforcing the kidney;and in the Song-Jin-Yuan Dynasties the treatment of diabetic nephropathy focused on nourishing kidney Yin and Yang,while,in the Ming and Qing Dynasties focused on nourishing qi and blood.The treatment of diabetic nephropathy in modern times focus on nourishing spleen and kidney,clearing heat and blood stasis,and clearing damp,reinforcing the kidney.Conclusion:There is no obvious difference in the principles and medication for the treatment of diabetic nephropathy between the ancient times and modern times,which focus on nourishing qi and kidney,clearing heat and nourishing Yin,clearing damp to reinforce the kidney,but the usage of specific medicinals is difference.In modern times The treatment of diabetic nephropathy is consistent for previous experience in ancient times and the research of modern medicine,the cogitation of Chinese medicinals of diabetic nephropathy is developed on the basis of heritage.
