Objective:According to pharmacists’requirement to manage diabetic patients,the real-time remote management system for diabetics was developed with Web languages and Microsoft SQL Server(2012 version)database technology,based on internet of things and cloud computing technology.With real-time data of blood glucose that diabetic patients delivered,pharmaceutical service was supplied to patients in real time with the goal of realizing integrated pharmaceutical caring services for patients with diabetes.Methods:(1)Medical specialists,pharmacists in hospitals and software engineers coordinated with each other to formulate the development process of the software.Management processes and module were developed according to the actual demand of pharmacists to manage diabetics.(2)Using two modes of real-time remote management system and routine follow-up,diabetic patients were managed for 3 months and indicators,such as self-management behaviors of diabetics,target rates of HbA1c,incidence of adverse events,were obtained.Then,the management effects of real-time remote management system were assessed on diabetic patients through the above indicators.Results:(1)The real-time remote management system for diabetic patients included“health perception terminal”,“cloud computing center”,“manual intervention and management backstage”and“health information service push system based on management of big data”.Through the successful automatic transfer of the real-time blood glucose value,active sending of warning text to manager’s phone by super-alert data itself,and function of“leaving marks”,the remote real-time management of diabetic patients was achieved.(2)Compared with the traditional mode of telephone follow-up,the remote real-time management system for diabetic patients could significantly improve the self-management capacity of patients,and patients were able to get more stable fluctuation of blood glucose,higher HbA1c improvement and compliance rate,which could help patients with hypoglycemia reduce the frequency of recurrence of low blood glucose.Conclusion:The real-time remote management system for diabetic patients developed by a hospital is characterized by more complete functions,more flexible ways of management,good extended performance.The study provides a management tool for pharmacists to render integrated pharmaceutical care to diabetic patients and the system will be further spread and applied.
