The knowledge, rules and process of standards for disease diagnosis and treatment recommended by the clinical pathway and the clinical guidelines can be introduced into the clinical diagnosis and treatment activity by the clinical intelligent decision support system(CDSS). It could make clinicians regulate their clinical behavior in accordance with the standards, enhance their clinical knowledge reserves, heighten the level of individual diagnosis and treatment, and thus reduce the incidence rate of medical accidents effectively. The low qualification of doctors and medical students are lack of clinical experience, cannot use the professional knowledge flexible and have few opportunities to take part in the clinical activities. According to their characteristics, combine the thought of computer-based case simulations with CDSS, a new kind of training oriented CDSS has been investigating. After the training study of simulation real case scenario, the low qualification of doctors and medical students can deepen the understanding the relevant medical knowledge in the clinical guidelines and more familiar with the diagnosis and treatment process of specific disease recommended by the clinical pathway, finally to achieve the purpose of improving individual clinical diagnosis and treatment ability. The present CDSSs always only support a particular disease, openness and extensibility is poorer and cases in the systems can't flexible expansion. In view of the above shortcomings, researching an open mechanism, which makes the CDSSs can support a variety of diseases and makes the cases in the case can expand flexibly, has the vital significance.This paper introduces the ideas of the computer-based case simulations(CCS) into CDSS. In order to make the system support the expansion of the different diseases and cases flexible expansion, a uniform way is needed to represent and store different diseases, a unified method to parse different diseases cases fast, and a tool to generate cases of different diseases quickly. Based on the above analysis, this paper studies a case description language to represent all kinds of diseases, definite a case file to store cases based on the language, studies a general case parsing pattern to parse different disease cases quickly, and studies a visual case designing tool to support rapid generation of different disease cases.First, the article introduced the concept, principle and related technology of CDSS. After that, the content of design and implementation of a system opening mechanism is introduced in detail, including case description grammar, case description file format, working process of the case parsing module and analysis designing and implementation of a visual designing tool. Then, the architecture and the function of each module of the clinical diagnosis and intelligent decision support platform and the process to customize a CDSS with the platform has been presented. Finally, by customizing different orthopedic training oriented CDSSs to verify the feasibility and effectiveness of the open mechanism proposed in this paper.The Experimental results show that by using the platform and related tools, people can quickly customize the training oriented CDSS of different disease with the same characteristics, with flexible expandability and openness. However, diagnostic and therapeutic features of different disease differ in thousands ways, the openness mechanism studied for different diseases in this paper is still partially open, and can only cover one kind of diseases of similar characteristics, the next study will focus on how to represent more complex, different disease cases.
