Objective: To analyze the correlation between the expression of Napsin A,p40 and the parameters of energy spectral CT in lung adenocarcinoma and lung squamous cell carcinoma,and to explore the potential value of quantitative parameters of energy spectral CT in reflecting histological types and biological characteristics of lung adenocarcinoma and lung squamous cell carcinoma.Methods: Sixty-eight patients with non-small cell lung cancer(including 36 cases of adenocarcinoma and 32 cases of squamous cell carcinoma)who underwent energy spectral CT scan and confirmed by pathology were collected.Multiparameter quantitative analysis was performed on two groups of patients by energy dispersive imaging.The effective atomic number and calcium content of plain scan lesions were measured.The enhancement degree? CT and the slope of energy spectral curve of the lesions were enhanced under the single energy of 70 keV.The expression of NapsinA,p40 in adenocarcinoma and squamous cell carcinoma(SCC)was detected by immunohistochemical PV two-step method.The expression of NapsinA,p40 in adenocarcinoma and squamous cell carcinoma of lung was analyzed and compared with the energy spectrum parameters of lung adenocarcinoma and lung squamous cell carcinoma.To evaluate the role of energy spectral CT in the diagnosis of non-small cell lung cancer and its biological characteristics.Results:1.Clinical data: there was no significant difference in clinical data(age,sex composition)between lung adenocarcinoma group and lung squamous cell carcinoma group(P > 0.05).2.Compared with the quantitative parameters of energy spectral CT: the effective atomic number of lung adenocarcinoma group and squamous cell carcinoma group were 7.86 ± 0.22,8.12 ± 0.44,respectively,the difference was statistically significant(P<0.05),and lung adenocarcinoma group is lower than the effective atomic number of lung squamous cell carcinoma.The calcium content of lung adenocarcinoma group and squamous cell carcinoma group were 3.10 + 0.92,4.51 + 1.54,respectively,the difference was statistically significant(P<0.05),and lung adenocarcinoma group calcium content is lower than the lung squamous cell carcinoma.The normalized iodine concentration(NIC)were 0.15 + 0.08,0.12 + 0.04,respectively,the difference was statistically significant(P<0.05),and lung adenocarcinoma group standard iodine content is higher than lung squamous carcinoma.The strong lesions on the degree of 70 keV energy(delta CT)were 31.38 + 14.77,28.21 + 21.35,respectively,and the difference was not statistically significant(P > 0.05),and the slope of the energy spectral curve was 1.63 + 0.63,1.91 + 0.38,respectively,the difference was statistically significant(P< 0.05),and the slope of the energy spectral curve of the lung adenocarcinoma group was lower than that of the lung squamous cell carcinoma group.3.Napsin A and P40 immunohistochemical expression comparison: Napsin A was negative expression in 32 cases of lung squamous cell carcinoma,35 cases among 36 cases of lung adenocarcinoma showed positive expression in varying degrees,only 1 case was negative.The positive rate was 97.22% and P40 was negative in all 36 cases of lung adenocarcinoma.The positive rate was 100% in 32 cases of lung squamous cell carcinoma.4.Correlation analysis of Lung adenocarcinoma group Napsin A,lung squamous cell carcinoma p40 expression and spectral CT quantitative parameters:The expression of Napsin A in lung adenocarcinoma group was positively correlated with the effective atomic number(r=0.431,P=0.009),calcium content(r=0.456,P=0.005),normalized iodine concentrations(r=0.348,P=0.037)and slope of energy spectral curve k(r=0.341,P=0.042)in lung adenocarcinoma group.The expression of lung squamous cellcarcinoma P40 and energy spectral CT effective atomic number(r=0.797,P=0.000),normalized iodine concentration(r=0.764,P=0.000),energy spectral curve slope of k(r=0.759,P=0.000)were positively correlated.There was a negative correlation between the expression of p40 and the content of calcium in the lung squamous cell carcinoma group(r=-0.458,P=0.008).Conclusion:1.The combined detection of Napsin A and P40 antibodies is of great significance in the diagnosis and differential diagnosis of lung adenocarcinoma and squamous cell carcinoma.2.The measurement of multiple quantitative parameters of the energy spectrum can help the diagnosis and differential diagnosis of the histological types of non-small cell lung cancer.3.In lung adenocarcinoma group,the effective atomic number,calcium content,standard iodine content of energy spectral CT and slope of energy spectral curve k were positively correlated with the expression of Napsin A,which could indirectly reflect the histological type,blood supply and differentiation degree of NSCLC.Prognosis and changes in certain microstructures of the tumor.4.In lung squamous cell carcinoma group,the effective atomic number of energy spectral CT,the standard iodine content and the slope of the energy spectral curve k were positively correlated with the expression of p40,and the expression of p40 was negatively correlated with the calcium content.The histological type of non-small cell lung cancer was evaluated by the correlation between the calcium content and the expression of p40.The changes of tumor biological behavior such as blood supply,tumor growth rate,metastasis and prognosis have some reference value.
