The Relationship Between Social Capital And Quality Of Life Based On Structural Equation Modeling (SEM) Among People Living With HIV/AIDS In Nanchang

Objective:To understand the current situation of social capital and quality of life among people living with HIV/AIDS?PLWHA?in Nanchang,and explore the relationship between them,in order to mobilize social forces to prevent and control AIDS,provide reference for AIDS care and assistance.Method:PLWHA were investigated from the Centers for Disease Control and Prevention?CDC?database of the Qingyunpu and Donghu districts of Nanchang from January to June,2016.Data collection consisted of a socio-demographic questionnaire,Personal Social Capital Scale?PSCS?and Medical Outcomes Study HIV Health Survey?MOS-HIV?.The Epidata3.0 was used to do the data input,and the SPSS 22.0 was used to do the statistical description,chi-square test,t-test,ANOVA and multiple linear stepwise regression,etc.The AMOS 21.0 was used to build a structural equation model to explore the relationship between social capital and quality of life among PLWHA.The level of significance was 0.05.Results:1.A total of 230 PLWHA were investigated,225 valid questionnaires were collected with answering rate 97.8%.126 PLWHA were in Donghu district and 99PLWHA were in Qingyunpu district in Nanchang.2.There were 184 males?81.78%?and 41 females?18.22%?.The average age was?41.39±15.16?,the age between 18³9 was highest?51.11%?.96.44%of the respondents lived in the urban,the single/divorced/widowed were 124?55.11%?,the education level in high school and above were 148 PLWHA?65.78%?,the respondents without income were 63?28.00%?,143 AIDS patients were employed?63.56%?.88.44%PLWHA through sexually transmitted infection,19.11%PLWHA were identity exposure,CD₄⁺cell count<200 were 101 PLWHA?44.89%?.3.The scores of bonding social capital,bridging social capital and total social capital were 14.11±3.40,9.46±3.43 and 23.68±5.55,respectively.Multiple linear regression showed that CD₄⁺cell count,income,marital status had significant effects on bonding social capital.Age and occupation had significant effects on bridging social capital.CD₄⁺cell count,Age,income,marital status had significant effects on total social capital.4.The scores of PHS and MHS were 44.25±5.10 and 42.93±5.08,respectively.Multiple linear regression showed that CD₄⁺cell count,income,occupation had significant effects on PHS.Marital status,income,CD₄⁺cell count,education level,age had significant effects on MHS.5.Bonding social capital and bridging social capital had impact on quality of life among PLWHA,the standardized path coefficients were 0.926 and 0.159.Trust network members?Cap3?,routine contact with network members?Cap2?,support by network members?Cap4?,resources possessed by network members?Cap5?had impact on bonding social capital,the standardized path coefficients were 0.958?0.921?0.919?0.531,respectively.Support by group/organizations?Cap9?,participate more often in group/organizations'activities?Cap7?,represent rights and interests among group/organizations?Cap8?had impact on bridging social capital,the standardized path coefficients were 0.951?0.924?0.637,respectively.Conclusion:1.The PLWHA had poor social capital in Nanchang,and the overall quality of life is better than PLWHA in parts of China.2.The more CD₄⁺cell count,income and married/cohabiting PLWHA,the more bonding social capital they will have.The younger and employed PLWHA,the more bridging social capital they will have.The more CD₄⁺cell count,younger,income and married/cohabiting PLWHA,the more total social capital they will have.3.The more CD₄⁺cell count,income and employed PLWHA,the more physical health they will have.The more CD₄⁺cell count,income,married/cohabiting,younger,low education PLWHA,the more mental health they will have.4.The structural equation model showed that,bonding social capital had large effect on quality of life,but bridging social capital had less effect on quality of life.5.In order to improve quality of life among PLWHA,we should make full use of bonding social capital,and develop bridging social capital at the same time.Government and relevant departments need intensify their efforts to help PLWHA,and strengthen coordination and cooperation among different departments and organizations,build up a number of AIDS care groups.Family members,friends,peers and neighbors should strengthen support for PLWHA,set up the mutual trust between each other,encourage PLWHA to participate in group activities.
