| ObjectiveTo understand the health resources allocation and health service ability of primary health care institutions in Shenyang.To analyze the satisfaction of residents,patients and medical staff to primary health care services in order to explore the shortcomings of the primary medical treatment and provide references of the medical development.MethodsThe data of the primary health care services situation were obtained by a survey of 2924 primary medical institutions in Shenyang in August 2015.The quality of the survey was ensured by specialists trained by the Municipal Health and Family Planning Commission.A stratified sampling method was adopted to collect the primary health care institutions,and the medical staff,patients and residents were randomly selected for satisfaction survey.Results1.There are 2924 primary health care service situations in Shenyang,including 254 community health service centers,114 township hospitals,2556 health clinics.The resident population in Shenyang was 8.30 millions.In 2014,the total number of diagnosis and treatment in primary medical and health institutions was 10.47 millions.2.There are 8510 staff in community health service institutions and township health centers,including 5134 staff members and 3376 extra staff members.Among these staff,the percentage of practicing or practicing assistant physician was 34.12%,registered nurses was 30.53%,general medical doctor was 1.03%.Among 7002 hygienic personnel,the percentage of junior and following titles was 60.70%,intermediate titles was 34.43%,senior titles was 4.87%.3.There were 3352 medical workers in the health room,including 2055 men(61.31%)and 1297 women(38.69%).There are 1885 people was secondary school education(56.24%),758 people was College education(22.61%),121 people was bachelor's degree or above(3.61%).4.There were 3894 beds in community health service institutions and township hospitals,the number of institution that reaches the standard was 64(25.20%)and 101(88.60%)respectively.Among 2556 health clinics,there were 1676(65.57%)standardized village clinics with an area of 60 square meters or above.5.The most common medical equipment in community health service institutions and township hospitals is electrocardiograph.The configuration of CR and DR is relatively small.The most common medical equipment in health clinics is sphygmomanometer.6.In 2014,community health service institutions were invested 531.72 millions,the township health centers were invested 329.58 millions.But from average inputs,the former is 2.09 millions,which is lower than the latter's 2.89 millions.Generally speaking,personnel training has the least investment.7.The satisfaction rate of medical staff was 75.09%,and the overall satisfaction score was 3.98±0.67.The satisfaction score of salary is the lowest.There was no difference in satisfaction among medical professionals with different technical titles(F= 1.650,P>0.05).There was a statistically significant difference in satisfaction among medical staff of different gender,age,education level,working years and occupation to primary health care institutions(P<0.001).8.The patient satisfaction rate is 88.60%,and the overall satisfaction score is 4.19±0.59,of which the lowest score is the tariff standard.There was no statistically significant difference in satisfaction of patients with different gender,occupation,education level,monthly average income and age to primary health care institutions(P>0.05).9.The satisfaction rate of residents is 82.76%,and the overall satisfaction score is 4.04±0.58.The lowest score was economic satisfaction.There was a statistically significant difference in satisfaction among primary residents in different occupations(F=3.386,P<0.05).There was no significant difference in satisfaction between residents of different gender,education level and age to primary health care institutions(P>0.05).ConclusionsThe number of primary health care institutions in Shenyang has not yet reached the corresponding standard,so we need to pay more attention to personnel training and equipment investment in primary health care institutions.Residents and medical staff had higher satisfaction with primary health care institutions.The dissatisfaction is mainly in salary or tariff... |
