BackgroundFemur head necrotic is a refractory and multiple disease in life,the incidence of disease is increasing year by year in our country.The primary cause of femoral head necrosis is local blood circulation disorder,The Clinically common femoral head necrosis is caused by blood circulation disorder caused by acute femoral neck injury and chronic alcohol intoxication,long-term use of glucocorticoid and smoking and other chronic stimulants can also cause femoral head necrosis.Patients with femoral head necrosis because the early symptoms are not obvious and the optimal treatment time is missed,the effect of routine therapy on advanced patients is not obvious,therefore,it is important to find a more effective treatment to improve the quality of the life of patients.In recent years,It has been widely used in the treatment of orthopedic disorders,such as rheumatoid arthritis,osteoarthritis,early healing of fractures,osteoarthritis,trauma recovery and loin and leg pain;Alprostadil injection is a prostaglandin drug,its main function is to expand blood vessels,reduce peripheral resistance,inhibits platelet aggregation and reduce blood viscosity,it has been applied to improve blood circulation disorder and antithrombotic therapy In recent years.Therefore,we observe whether the combination of lugua Polypeptide injection and alprostadil injection has better therapeutic effect on patients with clinical treatment of femoral head necrosis,to study the mechanism of the treatment of femoral head necrosis by using the femoral head necrosis model of adult rats,In order to improve the therapeutic effect and improve the quality of life of the patients.ObjectivesTo observe the therapeutic effect of lugua polypeptide injection combined with alprostadil injection on femoral head necrosis,to study the effect mechanism of the treatment of the femoral head necrosis in rats with the combination of lugua polypeptide injection and alprostadil injection.Methods1.Clinical treatment: We selected patients with femoral head necrosis that met the criteria of the study.Divided the patients into two groups: lugua polypeptide injection group,lugua polypeptide injection and alprostadil injection group(referred to as combination group).The treatment of lugua polypeptide injection group was treated by intravenous infusion of lugua polypeptide injection for 30 consecutive days and other routine treatment.The patients of combination group received 30 consecutive days of intravenous infusion of lugua polypeptide injection and alprostadil injection,the therapeutic effect was determined by the visual analogizing score method and the hip joint Harris scoring method,After treatment,to compare the symptoms of patients with femoral head necrosis in the two groups.2.Experiments on animals: The 32 adult healthy SD rats were randomly divided into four groups: the control group,no surgical operation;femoral head necrosis group,the right femoral head necrosis model was prepared but not treated;lugua polypeptide injection group,after the right femoral head necrosis model was prepared,intraperitoneal injection of 0.33 mg of lugua polypeptide injection and 1.5ml of sterile saline solution per day lasted 20 days.Combination group;after the right femoral head necrosis model was prepared,intraperitoneal injection of 0.33 mg of lugua polypeptide injection and 1.5ml of sterile saline solution containing 0.15 ?g prostaglandin alprostadil injection per day lasted 20 days.At the end of the treatment,the animal was taken out of the thigh bone for the following index detection: In each group,two femoral heads were placed in an ice physiological saline,and then centrifugation was used to detect alkaline phosphatase to determine the tissue activity of the femoral head necrosis;After the remaining femoral head was taken out,observation of the gross specimen,after fixation with 4% formaldehyde solution to prepare HE slices,the morphological changes of the femoral head were observed and the protein expression was detected by immunohistochemistry.Results1.Clinical treatment:It has therapeutic effect on patients with femoral head necrosis whether to use lugua polypeptide injection alone or lugua polypeptide injection combined with alprostadil injection,reduce the symptoms of femoral head necrosis.The combination of drug therapy was better for the patients than the individual use of lugua polypeptide injection.2.Animal experiments: There was no pathological change in the femoral head of rats in the control group;The morphological changes of the femoral head in the combination group were the lightest,the changes in the femoral head of lugua polypeptide injection group were worse than that of the combination group,but better than the model group;The content of alkaline phosphatase from high to low was the control group,the combination group,lugua polypeptide injection group and model group.ConclusionsClinical studies and animal experiments found that it is better to use lugua polypeptide injection combined with alprostadil injection than alone to use lugua polypeptide injection to treat femoral head necrosis,the mechanism for this outcome may be that alprostadil injection improves the blood supply and local microenvironment of the femoral head.
