Clinical Observation On The Effect Of Jianpi Bufei Huatan Decoction In Treating Bronchiectasis Stable Period (Pulmon Ary Spleen Qi Deficiency)

Objective:The purpose of this study was to observe the clinical efficacy of Jianpi Bufei Huatan Decoction in treating bronchiectasis stable period of lung-spleen-qi deficiency type,and further analyze the mechanism of Jianpi Bufei Huatan Decoction from both theoretical and clinical studies.We are trying to evaluate Jianpi Bufei Huatan Decoction' effect on alleviating bronchiectasis symptoms of stable period patients,improving patients' lung function,improving patients' quality of life.And we also hope to evaluate safety and adverse reactions of this treatment plan during the study.All in all,we are aiming at providing effective and safe long-term treatment plan from the perspective of traditional Chinese medicine for patients with stable period of bronchiectasis,we are also trying to explore the possibility of clinical application and practical promotion of Jianpi Bufei Huatan Decoction.Methods:A total of 43 patients who were selected from Department of Pulmonology at Hubei Provincial Hospital of TCM from December 2015 to December 2017.All patients enrolled in the study were required to meet the diagnostic criteria for stable bronchiectasis in western medicine and the syndrome differentiation standard for lung and spleen qi deficiency in traditional Chinese medicine.Then,43 patients were randomly divided into two groups,21 in the control group and 22 in the treatment group.The control group was treated with conventional western medicine: Eucalyptol Limonene and Pinene Enteric Soft Capsules.On the basis of this,the treatment group was treated with oral Chinese herbal de coction,Jianpi Bufei Huatan Decoction.After 30 days of contin uous medication,the scores of symptoms and signs,lung function related indicators,quality of life scores,and incidence of adverse reactions were compared between the two groups before and after treatment.Statistical software was used to analyze the data of this study.Results:1.Comparison of clinical effect between these two groups: The total effective rate of clinical effect in the treatment gro up(95.5%)was relatively higher than the control group(71.4%).The markedly effective rate in the treatment group(81.8%)was higher than that in the control group(52.4%).Differences between the two groups has statistical significance(P<0.05);2.There was no significant difference in TCM syndrome score s before treatment between the two groups(P>0.05).After treatment,TCM syndrome scores of both groups were lower than those before treatment(P<0.05).Furthermore,the improvement in the treatment group was comparatively better than the control group(P<0.05);3.Before treatment,there was no statistical difference in the forced expiratory volume in one second(FEV1),forced vital capacity(FVC),and diffusing capacity of lung for carbon monoxide(DLCO)in the lung function tests between the two groups(P>0.05).After treatment,the FEV1,FVC,and DLCO in both groups were increased(P<0.05);and the improvement in the treatment group was comparatively better than the control group,and th e difference was statistically significant(P<0.05);4.Before treatment,there were no statistical differences in respiratory symptoms,activity limitation,disease impact scores and SGRQ total scores between the two groups of patients in the St.George Respiratory Questionnaire(SGRQ)(P>0.05).After treatment,respiratory symptoms,activity limitation,disease impact and total SGRQ scores were decreased in the two groups(P<0.05);and respiratory symptoms,activity limitation,disease impact and SGRQ scores were lower in the combination of traditional Chinese and western medicine treatment group than in the western medicine control group(P<0.05).5.No adverse reactions occurred after treatment in the two groups of patients.No cases with abnormal laboratory indexes were observed after taking Jianpi Bufei Huatan Decoction and Eucalyptol Limonene and Pinene Enteric Soft Capsules.Conclusion:The results of the study showed that the combination of Jianpi Bufei Huatan Decoction and conventional Western medicine in the treatment of bronchiectasis patients in stable period of lung-spleen-qi deficiency type is generally safe and effective.The treatment group was not only better than the Western medicine control group in terms of clinical efficacy and improvement of symptoms,but also could effectively improve the patient's lung function,improve the quality of daily life of patients.No safety issues and adverse reactions found during the study which proved the safety of the treatment plan of this clinical rese arch.In theory,it is suitable for long-term use of patients in bronchiectasis stable period of lung-spleen-qi deficiency type,which will contribute to retard the progression of the disease.Paying attention to the maintenance treatment of patients with bronchiectasis during the stable period can help control the progress of the disease,reduce the number of acute exacerbations,reduce the frequency of antibiotic use,reduce the antibiotic resistance rate and adverse reaction,improve the quality of life of patients,reduce the economic burden on their families,and reduce the burden on social medical resources.In addition,modern medicine has no unified clinical guide line on the treatment of bronchiectasis stable period and clinicians also ignore the treatment of bronchiectasis patients during the stable period for a long time.Therefore,looking for a safe and effective treatment plan and taking the advantages of traditional Chinese medicine is very important and has far-reaching significance.The purpose of this clinical study is to investigate the efficacy and feasibility of Jianpi Bufei Huatan Decoction combined with routine Western medicine in the treatmentof patients with lung spleenqi deficiency syndrome of stable bronchiectasis.
