Objective:The First Affiliated Hos Pital of Yunnan University of Traditional Chinese Medicine in recent five years hospitalized in the first diagnosis of knee osteoarthritis(KOA,hereinafter referred to as)the basic situation and the treatment of traditional Chinese medicine and Western medicine were summarized,further combing KOA treatment rules and treatment were compared between four different Professional direction of the Department and to analyze.Methods : This study selected the first diagnosis of KOA in the First Affiliated Hos Pital of Yunnan University of Traditional Chinese Medicine from October 2011 to October 2016,which was first diagnosed as the medical record of hospitalized case number.According to the inclusion and exclusion criteria,some medical records were excluded.The original information,including sex,age,X-ray stage,use of Western medicine,distribution of TCM syndrome type,application of traditional Chinese medicine,application of Chinese medicine,analysis of curative effect,injection of joint cavity,acupuncture and massage,was collected.After completing the collection,enter the EXCEL table to build the database,and finally use the SPSS21.0 statistical software package to do the statistical analysis of the data.Results:1.The sex distribution of KOA patients in our hospital is much higher than that of men,and the proportion of men and women is about 1:2.13,and there is no significant difference between the Department of orthopedics and the Department of massage,Department of rheumatism and the Department of acupuncture and moxibustion,respectively(P>0.05).2.The age distribution of our hospital is 60-70 years old,accounting for41.2%,followed by 51-60 years old,accounting for 21%.There was no significant difference in age between the orthopedics department and the massage department,Department of rheumatism and Acupuncture Department(P>0.05).3.X-ray stage distribution,X-ray stage of KOA patients accounted for40.1% of stage III,followed by phase II accounted for 35.9%.There was no significant difference in the X-ray staging between the orthopedics department,the massage department,the Department of rheumatism and the Acupuncture Department,respectively(P>0.05).4.In the distribution of TCM syndrome,the main syndrome types of KOA patients in our hospital were qi stagnation and blood stasis(26.1%),deficiency of liver and kidney(21.6%),followed by damp heat obstructing collaterals(14.8%),phlegm and blood stasis(10.4%).There was no significant difference in the gender between the orthopedics department,the massage department,the Department of rheumatism and the Acupuncture Department,respectively(P>0.05).5.Treatment of traditional Chinese Medicine:?The main use of Chinese medicine for orthopedics is blood activating and blood activating analgesics,and the main massage department is tonifying the liver and kidney,removing wind and damp medicine,and the Department of rheumatism is mainly antirheumatic and divergent wind cold medicine,and the main acupuncture department is blood activating and blood replenishing medicine.There was a significant difference between the orthopedic department,the Department of rheumatism,the Acupuncture Department and the orthopedics department,respectively,(P<0.05).The use of prescription of traditional Chinese medicine on acupuncture and moxibustion department most accounted for 66%,and at least 44.9% Kezhanbi.? Hospital preparation : Bone injuries were compared with Tuina,Department of rheumatism and Acupuncture Department in the use of "bone mixture","Bushen Jian bone mixture" and "Qing Li Xiao Xiao Granule" in the hospital.The use of bony Bi mixture in the Department of orthopedics,the use of tonifying kidney and bone mixture in the Department of acupuncture and moxibustion,and the use of hospital clearing bone granules in the Department of massage were the most frequently used among the four departments.?Chinese patent medicine:The orthopedics department was compared with massage,Department of rheumatism and Acupuncture Department to compare the use of Tsai Huang Jian Gu tablet,Xuesaitong capsule(dripping pills),Tong Shu Capsule and Jintian Ge capsule.Department of Orthopaedics and traumatology in tenghuangjiangu,Xuesaitong capsule(pills),Jintiange capsules use frequency is higher than the other department.The frequency of massage used in Tong Shu Capsule was higher than that in other subjects.6.Oral Western Medicine:There were significant differences in the use of non steroidal anti-inflammatory drugs,calcium and cartilage protectant in the Department of orthopedics,Department of rheumatism and acupuncture and moxibustion department,respectively(P<0.05).7.Intraarticular injection : The massage department did not use joint cavity injection,and no comparison was made.There was a significant difference between the orthopedics department and the Department of rheumatism and Acupuncture Department for intra articular injection treatment(P<0.05).8.Acupuncture and moxibustion therapy:Acupuncture and moxibustion were not used in the orthopedics.The Acupuncture Department,the massage department and the Department of rheumatism use the most frequency of the acupoints: the knee,the crane top,the blood sea,the knee Yang Guan,the beam hill,the Yin Ling spring,the Yang Mausoleum,the Central Committee,the Zusanli and the Tai Chong.Meridian points to extrachannel points(35.9%),spleen meridian,stomach meridian of Foot Yangming(18.9%)(13.7%)mainly.9.Massage therapy : Tuina was not used in orthopedics,Department of rheumatism and Acupuncture Department.The massage department used the massage therapy,mainly using the one finger zen(15.8%),the roll method(15.8%),the double palm rubbing(13.4%),and the knee joint pull and extension loosening(13%).10.Health education and medical practice guidance:There were significant differences between orthopedics department,massage department,Department of rheumatism and Acupuncture Department in health education and medical practice guidance,respectively(P<0.05).11.The difference between the orthopedics department,the massage department,the Department of rheumatism and the Acupuncture Department in the days of hospitalization was analyzed by t test.The results showed that P < 0.05 had significant difference.The average length of stay in orthopedics department was less than that in massage department,Department of rheumatism and Acupuncture Department.There was no significant difference between the orthopedics department,the massage department,the Department of rheumatism and the Acupuncture Department in the daily average hospitalization expenses.P>0.05 was obtained by t test.Conclusion:1.The characteristics of our hospital and massage department,Department of rheumatism and Department of acupuncture and moxibustion treatment KOA for comprehensive treatment of traditional Chinese medicine and Western medicine,in the application of traditional Chinese medicine,Chinese medicine,western medicine hospital preparations and intra-articular injection,acupuncture,massage and other treatment methods reflects the focus and characteristics of each professional.The orthopedics department is more standardized in the treatment of KOA.2.Through the retrospective analysis of the medical records of the last 5 years,the non-surgical treatment of KOA in the Chinese medicine and hospital preparation,in order to treat qi stagnation and blood stasis,the deficiency of liver and kidney,and the application of the method of activating blood and dredging collaterals,such as the use of BJI in hospital.Oral administration of Western medicine combined with non steroidal anti-inflammatory drugs,calcium and chondroprotective agents can relieve pain,improve function and improve quality of life.3.The four departments have shortcomings on the basis of the above treatment,and there are still shortcomings in the use of Chinese medicine soup in the orthopedics department.In improving the clinical symptoms of KOA patients,we should learn the characteristics of Chinese medicine and acupuncture and moxibustion department to improve the curative effect.The four departments should take the early health education propaganda and medical training guidance as an important means to cure and consolidate the curative effect.4.The social and economic benefits of treating KOA in our orthopedics department are the best and the economic burden of patients is reduced.
