[Background]Pilon fracture refers to the distal tibial fracture of the tibial distance joint,The two or three pilon fractures are more severe,often comminuted,causing the joint surface to collapse and the ankle joint.Ankle joint is the weight-bearing joint of the human body,and its activity level directly affects the walking quality of the patient,lack of early and correct functional exercise after surgery,the affected limb can have muscle atrophy,tendon adhesion,joint stiffness and foot droop,etc.,which can affect the function recovery and seriously affect the normal life and work of patients.At present,the domestic type 2 and type 3 pilon fracture joint function rehabilitation exercise mainly adopts the traditional artificial massage and patients exercise actively,although this method can be improved to a certain extent of joint motion,but time-consuming,and exercise in patients with optional the gender is strong,not easy to stick to it.Therefore,how to solve the many problems of high complication and disability rate of pilon fracture,which is the focus of our study.[Objective]This paper discusses the role of periodic functional combination exercise in the postoperative rehabilitation of pilon fracture.Compare the difference between the two methods of rehabilitation exercise and provide standard operating procedure and nursing care plan for the recovery of pilon fracture.[Methods]This study during February 2016 to August 2017 in changzhou city second people's hospital,a total of four campus fracture ward of 59 patients with type 2,3 pilon fractures,line after surgery with the metihod of randomized controlled trials,patients were randomly divided into experimental group and control group,there was no difference between the two groups in the course of disease,symptoms,and inclusion criteria.In the experiment group,34 cases were performed in the second day after surgery:The first stage,the passive motion,with the apparatus exercise to drive the surrounding muscle activity,in case the joint is stiff,the muscle is weak;The second stage,the semi-active motion,the high elastic bandage help the exercise,gradually restored the ankle joint activity and the surrounding muscle strength;The third stage,the active movement,the metatarsal,back extension,internal rotation,and external rotation,strengthen the ankle joint activity and the surrounding muscle strength;In the fourth stage,anti-resistance movement,specific muscles and ligamentous strength and coordination exercises are carried out against the patient's plantar dorsal flexion.In the control group,25 cases were also performed in the second day after the operation:active exercise.In 7 days,15 days,three months,six months after the time line of two groups of patients with ankle-full score(AOFAS),ankle diameter measurement of motion(AROM)and ankle weeks,each group for the statistical analysis of the results.The comparison between groups was based on chi-square test or variance analysis,and P<0.05 was statistically significant.[Results]Compared with the control group patients,the experimental group 7 days,15 days after surgery in patients with ankle weeks narrow diameter significantly(P<0.01)difference was statistically significant,ankle-full score(AOFAS)after March 15 days after surgery,has improved significantly(P<0.01)difference was statistically significant,at the same time,the ankle of motion(AROM)in postoperative 3 months,6 months increased significantly(P<0.01)difference was statistically significant.[Conclusions]Ankle joint exercise combined operation of two or three pilon fracture postoperative recovery function can significantly improve the early metaphase swelling,alleviate pain,and promote function recovery.
