ObjectiveAnalysis of the sensitivity and specificity of the prenatal ultrasound screening for fetal structural abnormalities and ultrasound soft markers(detection rate)in 3 000 cases of pregnant women(single fetus),Analysis of the correlation in ultrasonic soft indexes and fetal malformation,strengthen the standardized ultrasound screening quality control system,make a standard for the application of ultrasonic soft markers,form the correct understanding of the system of prenatal ultrasound screening and ultrasonic soft indexes.MethodsFrom July 1,2013 to December 31,2014 3 000 pregnant women who accepted system of prenatal ultrasound in Tianjin Tanggu gynecology and obstetrics hospital were taken as research object.collected the clinical data of pregnant women,pregnant women were all told and check system of prenatal ultrasound screening examination and informed consent.The system of prenatal ultrasound screening according to the specification,storage standard sections and the abnormal section image,and measure the required data,reports,all the relevant images and data were saved more than three years.All cases were following up to 6 months after parturition.The sensitivity(detection rate)and specificity were analyzed to assess the significance of prenatal ultrasound screening and soft index.All the data was analyzed by SPSS17.0 software with chi square test.expected value<1 with exact probability method;1?expected value<5 With the continuous correction,The differences were statistically significant with P<0.05.Results50 cases of fetal malformations were confirmed in 3 000 cases of pregnant women with postpartum follow-up,fetal structural abnormalities accounted for 1.67%(50/3 000).42 cases were screened by prenatal ultrasound screening in the 50 cases of fetal malformations and were identified by neonatal surgery or pathology.the detection rate(sensitivity)was 84%(42/50)and the specificity was(2950/2950).There were 8 cases with single fetal malformation as False negative result with no fatal deformity,including simple ventricular septal defect in 2 cases,pulmonary valve stenosis in 1 cases,anal atresia in 1 cases,simple cleft palate in 1 cases,multi finger in 2 cases,and syndactyly of 1 cases.425 cases of fetal ultrosound soft marker wer e detected in 3 000 cases,which were confirmed fetal malformation in 30 cases,the incidence of ultrasound soft index was(425/3000),sensitivity of 60%(30/50),specificity of 86.6%(2555/2950).In the center,the specificity of echo intensity was 93.9%(2771/2950),and the sensitivity of choroid plexus cyst was the lowest(2/50)of ultrasonic soft markers.Ultrasonic soft markers and fetal malformations are associated with occurrence;more than 2 ultrasound soft index have a higher interrelationship with fetal malformations than single ultrasonic soft indexes of fetus.Fetal neck skin thickening,nasal dysplasia,intracardiac echogenic plaque,lateral ventriculomegaly,renal pelvis expansion,echogenic bowel,single umbilical artery had a correlation with the fetal structural abnormalities,choroid plexus cyst and fetal structural abnormalities had no correlation.ConclusionsThe normative system of prenatal ultrasound screening can effectively detect fetal structural abnormalities,avoiding serious death of fetal abnormality.objective evaluation system of prenatal ultrasound screening is necessary.Ultrasonic soft markers and fetal structural abnormalities have certain relevance,Plurality of ultrasonic soft markers of fetal structural abnormalities was significantly more relevant than that of single ultrasonic soft markers.To strengthen the correct understanding of ultrasonic soft markers,reasonable application of ultrasonic soft markers,can improve the detection rate of fetal malformation.The standardized system of prenatal ultrasound screening and quality control can not only screen for fetal malformations and also avoid medical risks.
