The Application Of Ultrasound Screening For Fetal Malformation In The First Trimester

Objective: To investigate the fetal structure malformation in the first trimester,analysis the relationship between the ultrasonic soft index and aneuploidy abnormalities,in order to improve the diagnosis level of fetal malformation in the first trimester.Methods: From December 2016 to August 2017,1441 pregnant women who were diagnosed by ultrasound in the second hospital of Hebei Medical University in the first trimester were selected as the object of this study,summarize the incidence of fetal structural malformation and the relationship between ultrasonic soft index and aneuploidy.Results:1551 fetuses were included in the study,of which 110 were twins.Gestational age 77-97 days,mean age 87.8 ±4.3 days old.(1)A total of 95 cases of structural abnormalities were detected in the first trimester,and the detection rate was 66.9%.The sensitivity,specificity,positive predictive value and negative predictive value of ultrasound screening in the first trimester for fetal structural abnormalities were 67%,99.9%,98.9% and 96.7%,respectively.(2)The IT increases with CRL from a median of 1.2 mm at a CRL of 45 mm to 2.5 mm at a CRL of 84 mm.IT was not clear of which is open spina bifida and anencephaly/exencephaly.(3)9 cases with superior mesenteric artery abnormalitywere diagnosed in the first trimester,in which there are 6 cases of omphalocele.(4)The risk of fetal malformation increased significantly with increased NT.Ductus venosus abnormality or unclear nasal bone showed a suggestive effect on fetal malformation.(5)Ultrasound diagnosis of twin chorionic is consistent with postnatal pathology in the first trimester,and ultrasound diagnosis 2 cases of conjoined twins anomalies and 2 cases of acardiac twins sequence,with the diagnostic rate of 100%.Conclusion: Ultrasound screening in the first trimester making the screening of fetal malformation window in advance,contribute to the early termination of the pregnancy serious deformity,reducing various injuries in the second and third trimester induced labor by pregnant women as far as possible.
