PurposeTo compare the intraocular optical quality between a refractive rotationally asymmetric multifocal intraocular lens(IOL)(SBL-3)and accommodating IOL(Tetraflex HD)in patients after phacoemulsification;To discuss the impact of position of near addition in the refractive rotationally asymmetric multifocal IOL on the intraocular optical quality.MethodsConsecutive age-related cataract patients with no contraindications had implantation of a refractive rotationally asymmetric multifocal IOL(SBL-3)or a accommodating IOL(Tetraflex HD),They were named MIOL(multifocal IOL)group,AIOL(accommodating IOL)group,MIOL-1 group.Of the 48 eyes,18 were in the MIOL group,20 were in the AIOL group and 10 were in the MIOL-1 group.The uncorrected distance and uncorrected near visual acuity,best-corrected diatance and best-corrected near visual acuity,distance-corrected near visual acuity were measured at 1 month,3 months after sugery.SR,MTFcutoff and objective pseudoaccommodation range were evaluated in 3 months by OQAS(Optical Quality Analysis System)postoperatively.Results1 months postoperatively,the distance-corrected near acuity in MIOL group and AIOL group were(0.39 ± 0.14)log MAR and(0.48 + 0.13)log MAR respectively,there are significant differences between the two groups(P <0.05).There are no significant differences in other visual acuity results(P>0.5).No significant differences between MIOL group and MIOL-1 group(P>0.5).3 months postoperatively,there are no significant differences in the uncorrected distance visual acuity,corrected distance visual acuity and corrected near acuity between groups(P>0.05).Ucorrected near acuity and distance-corrected near acuity(0.245±0.079 log MAR,0.279±0.074 log MAR)in MIOL group were better than the AIOL group(0.303±0.094 log MAR,0.339±0.077 log MAR),there are significant differences beyween two groups(P<0.05).No significant differences between MIOL group and MIOL-1 group(P>0.5).3 months postoperatively,the SR in MIOL group and AIOL group evaluated by OQAS are 0.19±0.29 and 0.15±0.05 respectively.The objective pseudoaccommodation range are 2.72±0.49 D in MIOL group and 2.19±0.49 D in AIOL group respectively.There are significant differences between two groups(P<0.05).No significant difference between MIOL group and AIOL group in MTFcutoff(P>0.5).There are no significant differences between MIOL group and MIOL-1 group in the SR,objective pseudoaccommodation range and MTFcutoff(P>0.5).ConclusionThere are differences between rotationally asymmetric multifocal IOL and accommodating IOL in intraocular optical quality.Position of near addition in the refractive rotationally asymmetric multifocal IOL do not significantly affect the intraocular optical quality.
